    This letter is on behalf of my daughter, _____ for religious exemption for the
    COVID-19 vaccination. I am requesting exemption for ____ as this vaccination
    conflicts with the free exercise of our religious beliefs which are rooted in our
    Christian faith. As requested, we are submitting additional information regarding
    exemption.

    The Bible teaches us to put our faith in the Lord and trust in him to keep us. Our
    personal religious beliefs includes our obedience to God’s law, and we believe that
    we are responsible before God for the the safety of our child, who was created by
    God.
