Dear [insert name]

Re: Request to reduced/different hours upon return to work from maternity leave

I am writing following our meeting on [Insert Date]. The meeting was arranged to discuss your request
dated [insert date], in which you requested that your hours of work be amended upon your return from
maternity leave.

We discussed your request for a change to your pattern of working to reduce your working week to
[Insert Number] days per week, following your return from maternity leave.

I am pleased to confirm that, optional [further to a trial period], your request has been granted.

[Insert details of arrangement that has been agreed].

Your salary and annual leave entitlement will be adjusted accordingly to reflect the different working
time.

We confirm that, as from [Insert Date], the terms of your employment contract will change on a
permanent basis. Please see enclosed two copies of your revised terms and conditions; one of which
you should return to me before [insert Date].

We agree that if you wish to revert to our original agreement we will try to provide full-time
employment in the same or similar status employment if circumstances allow a return to your previous
terms and conditions of employment, although we cannot guarantee that this will definitely be
possible.

Please note that we have treated your request as a flexible working request, therefore you will not be
able to make any further flexible working requests for the next 12 months from the date of your
original request.

If you have any further queries regarding the contents of this letter, please do not hesitate to contact
me.

Yours sincerely,
