    The undersigned is requesting for the availability of appropriate PCG vehicle to be


    utilized for the transportation of Twelve (12) PCG Trainees who will donate blood for
    the undersigned’s father’s operation at the Philippine Heart Center tentatively on 28
    July 2021.

    The trainees are intended to be picked-up from Coast Guard Non-Officers Basic
    Education and Training Center at Coast Guard Base Taguig en route to Philippine
    Heart Center, East Avenue, Quezon City and back on 260700H July 2021.
