AFFIDAVIT OF EXPLANATION

I, __________________________, of legal age, Filipino, single and a resident of #144

Liwayway, Sta. Rosa, Nueva Ecija, after having been sworn to in accordance with law, do
hereby depose and state the following:

1. That I am the legitimate daughter of Spouses _________________ and

______________ and I was born on June 11, 1991 at Casiguran, Aurora as proof by
the hereto attached copy of my NSO Certificate of Live Birth marked herein as Annex
“A”;

2. That I applied for a Philippines Passport using my correct Birthdate as June 11, 1991;

3. That unfortunately, while riding on the Plane I discovered that my registered year of
birth was 1990 instead of 1991, but I registered my correct date of birth, as proof of
the wrong entry I attached herewith my Philppines Passport as Annex “B”;

4. That I am executing this affidavit of explanation to attest the truthfulness of this fact
and to support my request with the Department of Foreign Affiars for the renewal of
my passport and to correct my Date of Birth as JUNE 11, 1991 and for any other
legal purposes it may serve.
