1) SITE QUALITY MANAGER
Responsibilities:
Primary responsibility in Site Quality Program Implementation in line with the Business
Owner requirements, such as;
 Set up the Quality System based upon ISO 9001
 Implements Quality System Training and Project Internal Audit.
 Follow-ups of Corrective Action for non-conformance.
Job Functions:
 Develop implement and maintain Project Quality Plan and Site Quality Program;
 Develop, implement and maintain other Quality related procedures;
 Implement Quality System induction, awareness, and communication program in
the project organization;
 Implement Project Internal Audit and follow up of Corrective Action Required for
non-conformance items;
 Monitor Contractor’s Quality performance through regular surveillance, audit and
reviews as appropriate;
 Attend Project Management/ Coordination Meeting (Weekly Meeting etc.) and
contributes to Project Management;

2) SITE QUALITY ENGINEER


Responsibilities:
Primary responsibility is Site Quality Program implementation that is delegated by Site
Quality Manager in line with Business Owner's requirements, such as:
 Develops and maintains the Site Quality System set up;
 Implements Quality System Training and Project Audit;
 Follow-up of Corrective Action for non-conformance and Preventive Action.
Job Functions:
 Implement and maintain Project Quality Plan.
 Develop, implement and maintain Project Audit Procedure.
 Develop, implement and maintain Quality related procedures.

 Review and monitor quality assurance programs.


 Implement Quality System Training in the project organization.
 Implement Project Internal Audit and follow up of Corrective Action Required for
non-conformance items.
 Implement Vendor/Contractor Audit as appropriate.
 Attend Project Management/ Coordination Meeting (Weekly Meeting etc.) if
required.
 Participate in Quality meeting.
 Prepare monthly Quality progress reports.
 Manage quality surveillance and inspection activities to ensure compliance with
requirements of Contract documents, Codes and Standards, Specifications,
Design Drawings, Project Procedures, Inspection & Test Plans, etc. and industrial
practices.
 Develop site office document control, communication and filing systems.
 Prepare Inspection Document Package for each type of inspectors, consisting of
document index, specifications, drawings, procedures, ITPs, matrix, registers etc.
 Establish line of communication with Contractor’s site Quality management to
ensure Contractor provides timely Quality updates, reports, records, notices, etc.
and for meetings/discussions on Quality issues and nonconformity resolution.
 Establish line of communication with contracted Inspection Agency offices to
ensure efficient management of inspectors’ mobilization, replacement,
demobilization and administrative matters, including verification of invoices and
timesheets.
 Prepare and provide Quality Induction to project personnel which should cover
Quality policy, objective, matrix, organization reporting/structure and overview of
project QMS. Also ensure contractor will carry out their own Quality induction.
 Ensure continuous implementation of Site Masterpiece Program by the Contractor
on all their sub-contractor at & off-site.
 Join develop and implement with Contractor the Quality promotion & campaign
such as awareness & performance notice board, appreciation/ recognition
scheme and flyers/ lesson learnt.
 Develop Quality Inspector Daily Request for Inspection (RFI) Assignment Form and
Quality Inspector Assignment Relocation Form.
 Authorize inspectors’ daily RFI assignment and engage with Project Quality
Manager for his authorization of inspectors’ relocation to other fabrication yards.
 Manage inspection activities by supervising Inspection Coordinators in assigning
RFI to inspectors, ensuring attendance and availability of inspection resources,
monitoring performance and approving timesheets.
 Design daily inspection report form, execute its implementation and review
inspectors’ daily activity reports.
 Supervise Inspection Coordinators in developing and maintain an RFI Register to
track status of RFIs, and attend to and resolve all outstanding issues identified in
the RFIs.

 Develop and maintain a Register of Client Request Items (CRI) or Quality Alerts for
Quality issues discovered at site, actions taken to resolve the issues and close-out
status.
 Develop and maintain a Non-conformity Report (NCR) Register to track NCRs
issued by PMT. Ensure Contractor submits Contractor’s NCR Register on regular
basis. Ensure all NCRs are closed in timely manner.
 Engage with Project Quality Manager for decision on Quality issues resolution,
acceptance of proposed corrective action for NCRs and closing out of NCRs.
 Engage with PMT and Contractor’s procurement, planning & control and
construction team for awareness of arrival of materials and equipment on site.
Ensure receiving inspection or surveillance activities are completed as indicated in
the ITPs. Ensure timely availability and compilation of Purchase Order, Inspection
Release Notes, Mill Certificates and Receiving Inspection Reports by Contractor.
 Establish line of communication with PMT and Contractor’s construction
representatives to create awareness of scheduled construction activities or
change in schedule that might affect PMT’s quality inspection and surveillance
works.
 Provide technical expertise in welding, NDT, inspection, testing, acceptance and
rejection of materials, equipment and workmanship.
 Participate in weekly progress meetings and bi-weekly Quality meetings at site
office.
 Perform bi-weekly Quality walkabout jointly with Contractor’s Quality
representatives.
 Ensure Contractor performs Quality Audits on site fabrication and construction
works at regular interval, and participate in the audits.
 Review Manufacturer Data Books for compliance and completeness.
 Review Contractor’s project procedures and other Quality documents and
records, and Contractor’s compilation of Project Final Documentation for content
and completeness.
 Review Contractor’s Test Packages, Mechanical Completion Punch Lists, Ready
for Sail Away Punch Lists and Ready for Start-up Punch Lists, to ensure compliance
and to provide Quality input.
 Ensure Contractor and Sub-Contractor compliance with the approved
Contractor’s and Sub-Contractor’s Project Quality Plan, including sufficient
resources to perform quality assurance and quality control surveillance and
inspection activities.
 Attend to Classification/Certification surveys, track issues raised by
Classification/Certification surveyors and ensure Contractor takes action to closeout
the issues.
 Provide support to PMT’s Construction, Planning & Control, Procurement and
Commissioning teams in Quality matters and Contractor’s progress and resources
verification.
 Develop and track project Quality KPIs based on input from Contractor and from
activities at site.
 Provide weekly and monthly Quality Progress Reports to Project Quality Manager.

3) SITE QUALITY SUPERINTENDENT / SUPERVISOR / COORDINATOR


Responsibilities:
Prime focus for quality control and is responsible for ensuring that the quality of the site
work performed by Contractor meets the quality requirements of the applicable project
specifications, standards, codes and regulations.
Job Functions:
 Review packages for adherence to Codes and assist the Quality Inspectors in
formulating checklists for inspection requirements.
 Review inspection & test plans for large packages, in accordance with the design.
 Develop planning for inspection manning and hold points in accordance with
project schedule.
 Join develop with contractor the control & monitoring registers for inspection
progress status.
 Control and monitoring of inspection & testing status by ensuring consistent
update from PMT Quality inspector
 Ensure qualification testing for special processes for the project.
 Ensure all Vendors and Contractors submit Inspection & Test Plans (ITP) for review
and approval.
 Ensure all ITPs have appropriate levels of “Hold”, “Witness”, “Monitor” and “Review”
points set for inspection activities.
 To ensure that all holds and witnesses are observed by the vendors/ Contractors.
 Ensure formal lines of communication are established for notifications of witness
and hold points (example request for inspection) activities.
 Ensure that the Contractors record and compile the inspection and test results.
 When required, to coordinate the inspection and testing activities from outside
resources and provide written assignment with specific responsibilities or Term of
Reference (TOR)
 Establish expectations for inspectors being utilised during the Project. When
necessary provide a written assignment
 Ensure qualification testing for welders is completed and a Welder’s Log is kept up
to date.
 Ensure Heat #s (of materials used) are recorded on weld maps for traceability back
to Material Test Reports and Purchase Orders.
 Ensure Weld Maps are up to date with details of Weld #, Welder’s ID, and location
of NDE.
 Monitor new installations or/ and any repairs, modifications to ensure that codes,
standards and specifications are met.

 Interface with the Site Quality Manager and Designers to resolve any issues,
problems and any non-conformance.
 Ensure Site Procedures for Material Receiving, Handling and Storage are followed.
Receiving Inspections are completed and documented.
 To prepare a daily surveillance report.
 Track and report key performance indicator for project QC.
 Prepare a brief close-out report for the Site Quality Manager on areas that went
well and areas that didn’t, for use in planning future projects – include
recommendations.

 Supervise welding quality assurance/control program.
 Ensure implementation of all welding & NDT procedures.
 Ensure maintenance of all NDT records, RT films and NDT interpretation/
assessment software if inclusive in the contract
 Ensure maintenance of all welding procedures and welder qualifications.

5) SITE QUALITY INSPECTOR


Responsibilities:
Responsible for quality control of installation activities.
Job Functions:
 Witness the inspection of all civil, architecture, steel structure, static & rotating
equipment installation, tank erection, electrical, instrument and etc.
 Confirm the inspection activities according to the applicable specifications or
manufacturer's instructions.
 Review Contractor's QC reports.
 Prepare Daily Surveillance Report.
 Confirm calibration record for inspection instruments.

 Compile and ensure thorough understanding of project AFC drawings,


specifications, procedures, ITP, shop drawings and applicable international codes
and standards, including AWS, API, ASME and ASTM.
 Ensure and confirm continuous/ consistent data update into applicable Control &
Monitoring Registers and provide input to Quality Superintendent/ Engineer
pertaining to Contractor’s Quality performance.
 Carry out and report consistently planned surveillance on various aspects of
compliance including manpower competency, skill workers performance,
Masterpiece program, reference documents by sub-contractors, Inspection,
Measurement & Testing Equipment, consumables, QC/testing record progress
etc.
 Witness materials and equipment receiving inspection or surveillance as indicated
in the ITP. Ensure availability and compilation of Purchase Order, Inspection
Release Notes, Mill Certificates (stamped by Contractor’s for compliance review)
and Receiving Inspection Reports by Contractor.
 Engage with Inspection Coordinator to track the up to date Request for Inspection
(RFI) notifications issued by Contractor and discuss assignment of task for
performing inspection in accordance with RFI.
 Plan to execute inspection for the assigned RFI by coordinating with Contractor
QC representative, search and make available the required drawings,
specifications, ITP and procedures, and organize in advance the required transport
and accommodation.
 Close each assigned RFI immediately after the inspection is completed by signing
off the relevant Contractor RFI/inspection result sheets and note inspection status
either accept, require re-work & re-inspection or reject, etc. Advise Inspection
Coordinator and Quality Engineer whether NCR or Client Request Item (CRI)/
Quality Alert (QA) needs to be initiated.
 In absence of RFI assignment, perform daily quality control surveillance of
Contractor’s works on spot/sampling basis to monitor compliance.
 Prepare and issue daily quality inspection & surveillance report to Inspection
Coordinator and Quality Engineer. Identify the inspection carried out during the
day by listing the RFI and advising their status and non-conformity/potential nonconformity
issues encountered and provide recommendation for solution.
Similarly, detail out the quality surveillance work performed in the same report.
 When assigned by Inspection Coordinator, participate in
Classification/Certification Society surveys. Issue to Inspection Coordinator and
Quality Engineer an inspection report detailing the findings by the
Classification/Certification Society.
 Witness Contractor’s Inspection, Test & Measurement Equipment (ITME) site
calibration when & where applicable. Ensure all 3rd party laboratory calibration
carried out with valid certificates and maintained by Contractor.
 Ensure proper use and care of Contractor supplied (become PMT properties)
Inspection, Test & Measurement Equipment (ITME). Ensure validity of calibration
certificates. Report damage equipment and tools and alert in advance of expiration
of calibration validity.
 Witness qualification tests as applicable for each inspection discipline and as listed
in ITP, specifications or codes and standards such as Welding Procedure

Qualification Tests and Welders Qualification Tests for Welding Inspectors,


Painting Qualification Tests for Painting and Insulation Inspectors, etc.
 Witness equipment erection/installation as applicable for each inspection
discipline and as listed in ITP. Ensure correct positioning, correct vertical and
horizontal alignment and all physical surface or paint damages are marked and
reported.
 Witness in process and system completion tests as applicable for each inspection
discipline and as listed in ITP such as Instrument loop tests for Instrument
Inspectors, hydrostatic tests for Piping/Mechanical Inspectors, equipment site
functional tests for Electrical/Mechanical/Rotating Inspectors and various system
completion tests.
 Ensure that Contractor properly marks, labels or quarantines rejected materials,
equipment or assemblies either of consumable, temporary or permanent nature.
 Gather up to date information on Contractor Key Performance Indicators (KPI) for
quality for each inspection discipline as applicable such as RFI cancelled, RFI
accepted, RFI rejected, NCR, rejection & repair rates, welder performance, etc.
 Assist Inspection Coordinator and Quality Engineer in preparation of PMT issued
NCR, tracking of NCR status and close-out. Perform inspection following
completion of corrective actions and provide recommendation for closing out of
NCR issued by PMT and Contractor.
 Establish line of communication with Contractor’s Quality and construction
representatives pertaining to change in schedule of construction activities that
might affect PMT’s Quality inspection and surveillance works.
 Participate in joint PMT and Contractor punch listing exercises prior to precommissioning,
mechanical completion and Ready for Sail Away (RFSA)
milestones.
 Perform punch listing walkabouts, review punch list reports and perform
inspection to ensure all open items are closed in acceptable and timely manner.
 Verify and confirm that pre-fabricated/ ship-loose items are properly prepared for
transportation to offshore or another site. The pre-fabricated/ ship-loose items
shall be physically inspected and to be consistent with the packing list approved
by discipline/ project engineer.
 Verify surplus materials are properly identified, stored and preserved for the
purpose of handover at the completion of the project.
 Participate in Quality Walkabout sessions when assigned by Inspection
Coordinator or Quality Engineer.
 Participate in Quality, technical, site progress or other project meetings/
discussions when assigned by Inspection Coordinator or Quality Engineer.
 Endorse the relevant fabrication, construction and pre-commissioning quality
records as evidence of review or attendance in compliance with project ITP.
 Review and provide input to project final documentation, including as built
drawings, and lessons learned when assigned by Quality Engineer.
 Provide QC input on Site Technical Queries (STQ) when assigned by Quality
Engineer.

 Support PMT Construction and Planning & Control Groups in providing input to
work progress status monitoring and Contractor resources and machineries count
verification.
 Compliance with the project Health, Safety & Environment (HSE) policies and
procedures, including the use of Personal Protective Equipment (PPE) and gate
passes.
 Monitor all safety aspects of fabrication, construction and commissioning
activities.
 Attend Contractor Tool Box Meetings prior to equipment erection activities and
other inspection related works as applicable.

6) SITE QUALITY INSPECTOR (WELDING & PIPING)


Responsibilities:

Job Functions:
 Supervise the welding work to maintain adequate welding quality.
 Witness inspection of welding for piping and others.
 Confirm inspection activities according to the applicable specifications.
 Check material identification of piping.
 Review Contractor's QC reports.
 Ensure that welding procedures, welders and weld materials are qualified per
project requirements.
 Ensure that the required welding records (e.g. procedure qualifications, welders’
qualifications, repair rates, weld/welder/consumables traceability, etc.) are
maintained in accordance with project requirements.
 Witness qualification of welders/ welding operators and continuously observe
their performance.
 Ensure that Contractor stores and issues welding consumables in satisfactory
manner
 Review Contractor’s personnel qualification and experience are satisfactory in
accordance project requirement and Contractor procedures.
 Ensure interpretation and recording of testing performed complies with project
requirements.
 Prepare Daily Surveillance Report.
