AFFIDAVIT

I, ZALDY C. BAHALA of legal age, married, Filipino and resident of

Sirang Lupa, Batangas City, under oath state:

That I am the president of Cooperative of Sirang Lupa Batangas City;

That in our registration in Cooperative Development Authority (CDA), our

Cooperative is registered as Cooperative of Sirang Lupa Batangas City, that
however in our Mayor’s Permit our Cooperative is registered as Sirang Lupa
Multi-Purpose Cooperative;

That said discrepancy was purely inadvertence and was not in any
manner committed to violate any law.

That I am executing this affidavit to attest the truth of the foregoing

facts and for whatever legal purpose it may serve;

That I am executing this affidavit to attest to the truth of the foregoing

and to prove Cooperative of Sirang Lupa Batangas City and Sirang Lupa Multi-
Purpose Cooperative refer to one entity.

That I executing this affidavit to attest to the truth of the foregoing and
for whatever legal purpose it may serve.
