REQUEST FOR INCLUSION OF FAMILY DETAILS OF PENSIONER IN PPO RESPECTING ARMY

1. With due respect and humble submission, I, the above named pensioner would like to submit the following
few lines for your kind consideration and sympathetic action at the earliest please.
2. I was enrolled in the Army on 28 Jun 1995, have been SOS from the Army on completion of the service limit
on 30 Jun 2019 (AN) and become pensioner with effect from 01 July 2019.
3. On scrutiny of my PPO (Pension Payment Order) No 174201900207, it has been noticed that my family
details (NOK name) are not mentioned in the PPO. However, the remarks erroneously shown as 'family
pension not notified being plural marriage case", which is not understood and needs clarification.
4. In this regards, a xerox copy of PPO and Annexure of Family details (sent while submission of advance
pension documents) is enclosed herewith for your ready reference.
5. It is also submitted for your kind information that demise of my first wedded wife had also been correctly
notified vide publishing Pt II Order on occurrence. Therefore, the documentation with respect to my family
details is correct as per Annexure quoted at paragraph No 4 above.
6. Keeping in view of the above, it is kindly requested that you may please examine the subject matter and do
needful at your end for early inclusion of my family details to avoid any monetary failure to the NOK (Next
Of Kin) at the later stage.
