Here is an example of the individual monitoring plan that I made for specific learning
area that needs immediate intervention. This monitoring plan aims to provide
intervention strategies and be able to track down the learner’s status. Some
adjustments are made along the process to further meet the learner’s need. The table
ANNOTATIONS shows that the Individual Monitoring Plan focused on the development of a Problem
Reader in English. It shows also that the providing supplemental reading materials
would be more effective with proper and regular guidance of the parent and the
teacher virtually. Secondly, it focuses on the weekly distribution and retrieval of
modules. It shows that communication is an approach to cheer the parents to
continue their commitment and engagement in their childs education amidst the
pandemic.

Here are some of the Weekly Home Learning Plan (WHLP) prepared from 1 st to 4th
ANNOTATIONS quarter. This will serve as a guide to provides the instructions on where to locate the
learning tasks in the SLMs and their specific learning instructions for each task. It also
provides a more precise instructions on how to accomplish the learners’ modules. In
the Learning Tasks the page numbers and learning tasks numbers are provided to
avoid misperception over the activity sheets. And lastly, the mode of delivery is the
column where you can see the guidance and reminders about the modules.

DLP for 2nd Class Observation

ANNOTATIONS Here are the two online class observation plans patterned to the DLP’S that I made. In
these lessons, I provided colorful pictures and in the motivation and lesson proper
because learners will surely enjoy the lesson. During the Class Observation the
alignment of the outcomes and the learning objectives were clearly emphasized. All
the activities and tasks done is there and it answer to the given objective.

I decided to make a PowerPoint and video presentation where I personally explained
the topic using simple yet exciting images, animations, and video clips to deliver a full
understanding over the lesson. This also brought back the excitement of the learners
to the subject in the absence of face-to-face instructions.

In this LDM Course at first was very difficult to work


on, but later on as the course goes on, I slowly understand
and get the point that this course can help us teachers to
improve in assessing ourselves and our way of delivering
teaching and learnings in delivering to the learners specially
in this time of Pandemic. This course also opened a lot of
IMAGE opportunities to teachers like me. This experiences and
learnings in the new normal gave me the wisdom, strength
and confidence to develop my IPCRF Portfolio efficiently
and effectively. In this way, I can strongly prove that I did
my best in teaching the learners because of the MOVs that is
present both in my LDM and IPCRF Portfolio.

Based on my reflection, it shows that I have a meaningful experience of


the things to improve my abilities and weaknesses. And as an educator
ANNOTATIONS all important things need to be documented properly. Yes, it is very
challenging to make this success, during the implementation of the LDM
2, I came to my realization of the things that we should do to give
students the quality education they deserve in spite of this Pandemic
and also improve our teaching skills and to adapt to the new normal.

ANNOTATIONS Here are some of my pupil’s quizzes and performance task. It shows here the level of
my pupils on their test’s outputs. In the assessment part, I asked the parents to help
their child and guide in answering the activities so they can also motivate and
reinforce their children on what to do on teaching their child. Monitoring their learning
progress is a part of evaluating them. Delivering learning interventions are done
weekly and quarterly.

Here are some photos of pupils and parents embracing the new normal of education.
It shows the distribution of milk feeding to parents for their children and distribution
and retrieval of modules which confirms that they are willing to support the learning of
their child amidst the pandemic in every aspect.
ANNOTATIONS This school year will not be possible without the help of the parents/ guardians in
guiding their child along the process of modular distance learning. I conducted some
meetings with the parents to aid their queries about the modules and to give
additional instructions on how to accomplish the learning tasks given to their children.
