SCHOOL AND PARENT AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This agreement made entered into and between:

The Teneguiban National High School with office address at Salvacion, Teneguiban,
El Nido, Palawan, herein represented by CRYSTINE L. TANALEON, Teacher-In-Charge
referred to as the FIRST PARTY;

-and-

The Parents of Teneguiban National High School learners for the School Year 2020 –
2021 referred to as the SECOND PARTY;

-witnesseth-

WHEREAS, the FIRST PARTY is a national government agency vested with

authority, accountability and responsibility for ensuring access in promoting equality and
improving the quality of basic education amidst the COVID – 19 pandemic;

WHEREAS, the SECOND PARTY is the known biological parents, legal guardians
of the learners officially enrolled at Teneguiban National High School with full responsibility
in acquiring quality basic education of their child;

WHEREAS, the FIRST PARTY is responsible in providing correct and complete set
of Learning Materials for the education of every learner through Modular Distance Learning
Delivery;

WHEREAS, the SECOND PARTY is the primary partner of the school in the New
Normal setting by providing gadgets (Tablet, Cellphone) and allowing the learners to utilize
technology as a medium of the learning process through the use of Digital Modular Distance
Learning Delivery;

 WHEREAS, the FIRST PARTY as authorized by the law shall monitor the
educational progress of the learners for immediate and appropriate interventions as the case
may be;

WHEREAS, the SECOND PARTY in compliance with the guidelines of Modular

Distance Learning Delivery shall continuously communicate to the FIRST PARTY for
giving feedbacks in top of learners’ progress.

This agreement shall take effect immediately upon signing hereof, maybe amended or
modified upon mutual agreement by both parties and shall automatically terminate upon
satisfactorily fulfilment of all terms and conditions embodied hereof.
