Neonatal sepsis

Definition
Life threatening condition due to dysregulated host response to infection
Clinical response- how body response to infection
SIRS – body systemic response
Can be due to pathogen- bacteria, fungi , parasite, virus
and other , trauma, burn, pancreatitis

Cytokine storm due to infection


TNF , interferon released (inflammatory agents)
SIRS > Sepsis > Severe sepsis > Septic shock
Sepsis (SIRS plus infection)
Severe sepsis( Sepsis with cvs dysfunction, ARDS or >2 organ)
Septic shock
Lactate – byproduct of anaerobic respiration , due to poor perfusion
SOFA score – monitor patient progress response to treatment
Qsofa -respiratory rate >22, change mental status, systolic less then
100mm/Hg
Cytokines – cause inflammation ,endothelial barrier dysfunction, coagulation,
clot formation
S- shivering, fever,
E- extreme pain, discomfort
P- pale, discoloured skin
S- sleepy confused
I- I feel like die – cardiorespi function decline
Shock- circulatory dysfunction -failure to deliver sufficient oxygen and other
nutrients
Septic shock
Early resuscitation, antibiotic, maintain perfusion , eliminate pathogen
