AFFIDAVIT OF SOLO PARENT

I, ELSIE O. MANUBAG, of legal age, Filipino, and presently residing at Libertad, Kauswagan,
Lanao del Norte, after having been duly sworn to an oath in accordance with law, do hereby depose and
state:

1) That I got married to my husband DELFIN L. MANUBAG in 1997 however, said marriage did not
prosper and instead, the both of us started to live separate lives since 2001 up to present;

2) That we had a son named “JED ANGELO O. MANUBAG who was born on May 8, 1999;

3) That since my de facto separation with my estranged husband, I have become both the mother and
father to our son JED ANGELO and I singlehandedly had to support my family on my own.
Consequently, because of this personal circumstances, I was qualified by the Municipal Social Welfare
and Development Office as a Solo Parent with Solo Parent ID No. 17-002;

4) That I am executing this affidavit to attest to the veracity of the foregoing statements, and for any and
all legal intents and purposes this may serve best.

IN WITNESS WHEREOF, I have here unto set my hand on this 29th day of April, 2019 at
