BOARD RESOLUTION NO. 2020-___

Board Authorization for President Roldan “Jao” Clumia,

and/or his duly designated substitute, to represent the
Union as signatory, and union representative relative to
a Chinabank Account with Account No. 1123-00-00037-
4.

During the Board Meeting held on __ May, 2021, at the __________,

St. Luke’s Medical Center 279 E. Rodriguez Sr. Avenue, Quezon City, the
following has been discussed and approved:

WHEREAS, on 07 May 2021, the Union received a Notice of Account

Inactivity from Chinabank, relative to an account in the name of St. Luke’s
Medical Center Employees’ Association with Account No. 1123-00-00034-4.

WHEREAS, the said notice informed the union that “accounts will be
considered dormant if there has been no activity for two years for savings
account and one year for checking accounts.”

WHEREAS, upon confirmation, the union office does not have any
records of the aforementioned account, nor was the details of the account
indorsed to the union by the previous leadership.

WHEREAS, to ascertain that there is no possibility of misappropriation

and malversation of the Union’s funds, it is imperative that the Union
learns of the history of the aforementioned account.

THEREFORE, IT IS HEREBY RESOLVED, as it has been resolved, that

SLMCEA President Roldan “Jao” Clumia, and/or his substitute, is hereby
AUTHORIZED and APPOINTED as the Union’s Representative to handle and
represent the Union as both its representative and signatory relative to the
Chinabank Account in the name of St. Luke’s Medical Center Employees’
Association with Account No. 1123-00-00034-4.

RESOLVED FURTHER, that as part of this authorization, the Union

President shall request for the Statement of Account of the aforementioned
Chinabank Account.

RESOLVED FURTHER, that the Union shall investigate and ensure

that union funds were not used for any other purpose other than its
intended use.
