survey respondent, or if you are taking
the survey personally as an individual
representing yourself
• Download the sections of the survey on
the right side
• Scroll through all of the questions to learn
what information you need
• Gather that information before you start the
survey
• Some of the questions require actual data to
be helpful; in other questions a best estimate

Achievement Rates
Defining Tiers
• Results for each question are presented as tiers to help
organisations prioritize their practice advancement
• Tier Zero – below minimum standards
• Tier One – good hospital pharmacy practices; support the safety of
procurement, preparation, distribution and administration no matter the
available resources
• Tier Two – the start of clinical service activities which address
appropriate use but do not serve every patient due to available
resources
• Tier Three – robust clinical services and integrated information
technology systems in place to serve every patient

achieve ideal pharmacy practice (Tier Two-Tier
Three) in as many categories as possible.
