Dear Mr. Macayan:

I am Eugenio B. Alfante Jr., an employee of Lapanday Foods Corporation which has plantations
in Region XII.

I am the Manager of the Information Technology Solutions & Services of Lapanday Foods
Corporation.

I am writing LGU Kiamba for my intention to re-enter our plantation located in Kiamba
Municipality which is part of my area of responsibility.

In this connection, I would like to request an approval for re-entry on November 27, 2020 for 1
day only in accordance to your Covid 19 protocols and guidelines as follows.

1. A formal notice will be sent to LGU 2 days ahead before arrival attached with an
itinerary informing the purpose. (Duly approved by the Mayor of Kiamba).
2. Maintain social distancing with subordinates, at least 2 meters distance.
a. Only Plantation manager/ Engineering manager/ QA manager/ Admin and HR
manager/ Sigatoka manager will be our main contact with every visit.
3. Must wear FACE SHIELD (Mandatory) and MASK. No face shield, no entry as
instructed by Health office.
4. I will visiting the farm for 1 day only (Kling farms)
5. NO MEETINGS / NO DINE IN inside the staff house / office or else all Kling
personnel in the office / cook will be quarantined for 14 days
6. Coordinates with your COVID -19 task force upon my arrival in Kling.
A. You may accompany me with any of your representative of LGU covid19 task
force during the farm visit

Purpose:

1. Conduct aerial footage of the entire Plantation

2. Check IT facilities (Routers, Personal computers, Networks, Weather Station).

Herewith the itinerary and contact persons during my stay in the plantation.
