Pilapil St. San Miguel, Pasig City, after having been duly sworn in accordance with
law, hereby depose and say:

1. That I am the grandmother of JOAN FRIZZA VELASCO MALONZO

who is an overseas worker;

2. That I am attaching herewith a photo copy of her Certificate of Live

Birth for authentication.;

3. That I hereby attest to the authenticity and genuineness of the said

document ;

4. I am executing this affidavit to attest to the truth of the foregoing

allegations and for the purpose stated above.
