unit of PISTOL VPR calibre 9mm with serial number 12395 with two magazine and 12
pieces of ammunitions.

Gun Owner: Miguel Oledan y Homeres, deceased

Janina Oledan y Eco, wife of the gun owner, of legal age, pensioner,
residing at B11 L20 Babilonia Villa Nova Subdivision, Brgy. Nagkaisang Nayon,
Novaliches, Quezon City.

Personnel Involved: PSMS Democrito Espinosa Sub-Station 1, Brgy.Nagkaisang

Nayon Focal Person and PEMS Brian Viola, RSPNCO

6. Why/How:
Mrs. Janina Oledan y Eco, assisted by the above-named police officers,
personally appeared before this station and voluntarily surrendered/deposited the above
describe firearm of her husband to PEMS Brian T Viola, RSPNCO of this Station for
safe keeping. Said handgun will be released to the registered owner upon renewal of
the firearms license at Firearms and Explosive Office, Camp Crame, Quezon City.
Attended by PSSg Jonathan Marciano duty investigator.
