DESIGNATION AS SCHOOL LIBRARY HUB COORDINATOR

1. You are hereby designated as School Library Hub Coordinator effective

June 10, 2021.
2. The SHLC roles and responsibilities are as follows:

a. Received learning materials such as the SRMs in the D-Library Hub;

b. Facilitate the distribution of the reading materials to the learners in
the or area;
c. Conduct an inventory report and coordinate with the division
librarian of any discrepancies of the delivered materials and untoward
incidents like damage and lost or unreturned books;
d. Perform other tasks related in support to the conduct of various
programs and activities that promote reading and library services;
e. Signing of exit form or clearance of teachers and other non-teaching
personnel for the school library.

3. Your designation shall remain in effect unless sooner revoked, amended

accordingly or until the assumption of another coordinator is determined by
the undersigned.
