INFORMED CONSENT LETTER

Dear Respondent,

I’m Jasmin P. Paradiang, a Grade 12 student here in Mary Immaculate School,Inc. I am conducting a study

entitled “The Effects of Online Class on Financial needs of Grade 10 St. Paul Students of Mary Immaculate

School Year of 2020 – 2021”. The objective of this study is to know the effects of online class with regards to

your financial needs. The tool being used is Revised Student Survey. The effects of Online Class will be identified

just after the data are analyzed. By doing so, I have a survey that I need your cooperation. So, I enclose here with a

copy of this letter that consists of survey questionnaire. All items are asking about your feelings and thoughts to

your online study for the whole semester. Your participation is voluntary. Filling in the survey indicates that you

have read the information and accept and to participate in this study. Your response will be private and

confidential. Please answer all the parts accordingly by following the instructions.
