2015 at Hagnaya, San Remigio, Cebu. Copy of her Death Certificate is hereto
attached as Annex “A” and made part hereof. And that the names and
addresses of her legal heirs are as follows:

a) ALONA O. SABORDO - Hagnaya, San Remigio, Cebu;

b) JESUS P. OCIO, JR. - Hagnaya, San Remigio, Cebu;
c) ALJUN M. OCIO - Hagnaya, San Remigio, Cebu.

It is therefore prayed that the foregoing legal heirs be ordered substituted

as party-plaintiffs in this case.
