Self-Declaration by TCS Associates resuming office

COVID-19 has spread to several countries in a very short span of time. This
necessitates all organizations, including TCS, to prepare itself to ensure the safety
of all TCSers. This declaration from you will help TCS to plan the arrangements at
the work location for your safety and safety of your colleagues.

Your safety is our topmost priority and we assure you that we have taken every
precautionary measure as you resume office. Every effort is being taken to ensure
a safe environment, be it during your transit or in office premises. Regular
sanitization of the premises is carried out and everyone present is required to
adhere to precautionary measures while working. Medical facility at premises are
operational. Social distancing, thermal screening, health check-up of our contract
staff, self-declaration, food at desks and other stringent measures adopted by the
organization will continue to maintain a healthy work environment.

Associate Declaration

I understand the risk of COVID-19 infection through contact with any person
(including family members, close contacts) who have been tested positive to
COVID-19 infection or is a potential suspect for COVID-19. I also understand that
symptoms of fever, cough, respiratory distress may indicate infection. I am aware
that I need to take strict precautionary measures to protect myself, my family and
my colleagues when I commute to and from office. I am aware about the current
situation of COVID-19 and the lockdown/restrictions within the country imposed
for hotspots and containment zones.

I confirm the following to enable TCS to make the required arrangements for me to
work in the Office premises and take precautionary measures to protect me and
everyone in the Office premises.

 I have verified my current address, contact number, and emergency contact

number in the system and corrected it wherever necessary to keep it current
 I have installed the Aarogya Setu app on my mobile and updated it with
correct information
 I have not been under a COVID-19 quarantine or if I had been, either on
request of the Government authorities or by TCS, then 14 days have passed
after start of quarantine
 I have not travelled to/transited from any international location or locally
known COVID-19 cluster, or if I have, then 14 days have passed after my
travel
  To my knowledge, in the last 14 days, I have not come in close contact with
any COVID-19 positive person or a potential suspect for COVID-19,
including my family members or persons in the community.
 I am currently not residing in a "Containment Zone" as identified by the
State Government
 I understand that any medical conditions or co-morbidities (eg. Diabetes,
hypertension, heart ailments, lung ailments, pregnancy, etc.) puts me at a
higher risk.
 My work requires services to be delivered from the Office premises and I
am WILLING to come to the Office premises for discharging my duties.

By submitting this self-declaration, I agree for TCS to process and store this
information as per specific requirement to contain COVID-19. I also understand
that details may be provided to legal authorities for any statutory purpose, if
needed.

I hereby declare that the information given above is true to the best of my
knowledge and belief and if found untrue, I shall be subjected to disciplinary
action by TCS. It is my responsibility to inform my HR/Supervisor in case of any
changes in the details furnished in the survey.
