REQUISITION LETTER FOR PREPARATION & FILLING OF INCOME TAX RETURN


(SALARIED INDIVIDUAL)

TAX YEAR: 2021

Dear Sir / Madam,

This is to inform you that the income tax return along with wealth statement & its reconciliation for
the subject year is due for filing by September 30, 2021. With reference to the below mentioned
heads of income, as applicable, you are requested to please provide the following documents /
information, so as to enable us to prepare / finalize the income tax return along with wealth
statement & its reconciliation and file / submit to the tax department within the prescribed time i.e.,
September 30, 2021.

1. 1. Salary Income

 Detailed breakup of salary, allowances & other perquisites / receipts, pay leaves, payment in lieu

of leaves, over time payment, Bonus, Commission, Fees.

 Detail of House rent allowance, Rent Utilities, education, entertainment, and travel allowance,

any reimbursement of expenses by the Employer.

 Detail of Any amount received as the consideration of entering into employment relationship

other than salaries and wages.

 Details of Termination Benefits whether received voluntarily or under an agreement, Pension

and annuity or any supplement to pension provided by the Employer.

 Detail information of Vehicle(s), loans, accommodation provided by your employer, if any.

 Provident fund / Gratuity fund statement of account, and work condition supplements if

applicable.

 Shares issued to employees under Employee share option scheme, restrictions, if any, along with

fair market value at the date of issue and consideration paid.

 Salaried persons are required to file the return electronically, therefore, login ID and password

of e-portal may also be provided, if changed.


2. Other Additional Information

 Bank statement(s) of all accounts maintained by you during the subject year

 If the Taxable Income of an individual is less than Rs. 1,500,000 per annum, the vouchers of

children’s school fee submitted during the tax year 2019.

 Zakat paid / deducted, if any.

 Donation to charitable institution, if any.

 Detail of interest paid on house loan during the Year, if any.

 Contribution to different funds, if any.

 Any other relevant information, if any.

4. Wealth Statement
 Detail of assets & liabilities held by you as at 1st July, 2020 along with copies of their
ownership documents [if not provided before].
 Details of during the year movement in the following heads.
 Agricultural Property (indicate Unit no and Location) ownership documents.
 Commercial, Industrial, Residential Property – (Purchase and sales of property
along with transfer documents)
 Investment (Account / Annuity / Bond / Certificate / Debenture / Deposit / Fund /
Instrument / Policy / Share / Stock / Unit, etc.) - (Indicate Form, Institution, and
Currency)
 Debt (Advance / Debt / Deposit / Prepayment / Receivable / Security) - (Indicate
Form, Debtor's name, CNIC)
 Precious Possession (Antique / Artifact _ Jewelry / Ornament / Metal / Gemstone) -
(Indicate Measurement Unit, Quantity).
 Cash and Bank Balance (Savings, and Current account Banks Statements)
 Any other Assets not stated above.
 Gift (Provide the Gift deed along with transfer documents)
 Inherited Assets – along with inheritance deed.
 Detail of proceeds realization certificates in case of foreign remittances
 Credit (Non-Business) (Advance / Borrowing / Credit / Deposit / Loan / Mortgage /
Overdraft / Payable) – (Indicate the Institute name or Persons CNIC along with
documents)
 Assets held in the name of spouse / minor children.

Furthermore, it is pertinent to note here that as per section 182 of the Income Tax Ordinance, 2001, any
person who fails to file the return by the due date the minimum applicable penalty has been increased
from Rs. 20,000 to Rs. 40, 000.

It is also important to mention here that in finance bill 2021, tenth schedule has been inserted which states
that the withholding tax rate for the persons not appearing in the Active taxpayers list would be subject to
withholding income tax at the 100% higher than the rate applicable to persons appearing in Active
Taxpayer List.

Any delayed provision of data may result in delaying filling of income tax return and our team will make
its best effort to file all tax returns within due date, however, preference of filling of return will be given to
clients who will provide information at the earliest, hence we will not be responsible for delaying in filing
of return where data shall be provided late by the client.

We would appreciate your due co-operation in providing the information at the earliest, which would
facilitate us to provide proper attention to your case & finalization of income Tax Return along with
Wealth Statement & its Reconciliation within stipulated time period i.e., September 30, 2021.
