I, Marlen C. Corpuz, Filipino, married and of legal age, with address at Sitio Bana, Pinaod, San
Ildefonso, Bulacan Philippines, after being duly sworn according to law, hereby depose and state
that:

1. I am known by the name “Marlen Corpuz” and have used the same in my personal
records and interactions, as shown by the following (a copy of which is hereto attached as
Annex “A”):

Philippine Passport

2. My name was recorded on the following as “Marilyn Corpuz” (a copy of which is hereto
attached as Annex “B”):

Transfer Certificate of Title No. 11794

3. Both the names “Marlen Corpuz” and “Marilyn Corpuz” Appearing in the Transfer
Certificate of Title No. 11794 refer to one and the same person, the Affiant.

4. I am executing this affidavit to attest to the truth of the foregoing and for whatever legal
purpose it may serve.

JOINT AFFIDAVIT OF TWO DISINTERESTED PERSON

We, Jose I. Turla, and Engracia D. Rivera, Filipinos, both of legal ages, and with
residences at Valentine St. Blk.1 Grp 1 Brgy. Southside Makati City, and 841 Nueve de Pebrero
St., Brgy. Mauway, Mandaluyong City Philippines, after being duly sworn according to law,
hereby depose and state that:

1. That we personally know the person of Marlen Corpuz, married to Reginal Corpuz and
was a former resident of 841 Interior 3 Nueve de Pebrero St, Mandaluyong City.

2. That she was one of the original recipients of the deeds to ownership of land awarded by
the local government of Mandaluyong to its underprivileged constituents under RA 7279.

3. That however, upon receipt of Transfer Certificate of Title no, 11794 her name whose
said certificate was registered to appeared as “Marilyn Corpuz”.

4. That the name Marilyn Corpuz in the Certificate of Title no. 11794 and the Marlen
Corpuz we personally know are one and the same person.

5. That we are not related to the said person either by consanguinity or affinity.

6. That we execute this affidavit to attest to the truthfulness of the foregoing statements and
for whatever legal purpose this may serve under the law.
