Subject: Inquiry regarding as to why retirement pension of beneficiary has not been given

To whom it may concern,

This letter is in regards to inquiry about pensioner Mr. Kian Kuo Ong, whose SSS information, a
recent bank statement from UCPB Libertad Branch, Bacolod City, and his deceased wife info are
shown below.

It would be really grateful if you can let me know the details as to why Mr. Kian Kuo Ong has not
