This is to certify that I have personally inspected the premises and examined the various areas of
the building from internal to external along with compound walls with a view to ascertain the Structural
Stability of the building. The Structural elements inspected included columns, beams, slabs and trusses
of each bay.
As per site inspection conducted, I have found out that the above mentioned subject has
complied and conformed to all the provisions of the NATIONAL BUILDING CODE OF THE PHILIPPINES
and other local existing codes governing building design, construction and safety.
Furthermore, design wise, all the primary and secondary structural members of the above
mentioned subject show structural integrity and stability with outmost soundness and safety, all
features and appearance of structure are in suitable condition and can be adopted for future use.
I therefore certify that the said building structure is safe and suitable for the building occupants
as mentioned in this stability report.

EVALUATION REPORT
I. INTRODUCTION
This report represents the result of the structural investigation conducted at the INDUSTRIAL
WAREHOUSE/PRODUCTION WAREHOUSE owned by Mr. Prescilo Lazaro and located at Sta. Cruz, Santa
Maria Bulacan. Consequently, an inspection was performed and investigative analysis was conducted
based on the data gathered from the existing structural plan and details.
II. OBJECTIVE
The purpose of the structural investigation is first; to evaluate the new overall capability of the
existing structure in accordance with National Structural Code of the Philippines and the other related
structural and building codes. Secondary is to recommend practical and economical corrective measure
to warrant the intensity of structural stability and soundness.
1. Columns
- The columns re sufficient to carry all the vertical loads and no Structural stress found on
columns.
2. Beams
- The reinforced beams are likewise sufficient in carrying the vertical loads and no deflection
found.
3. Truss and Rafters
- No visual evidence of deflection and sagging found on the trusses and rafters.
4. Walls
- No Cracks found on the walls and structural bends.
V. Conclusion
Based on the results and recommendations, it can be concluded that the structural members
can safely carry their designated axial and non-axial loadings hence, no corrective measure are needed
and the structure is certified structurally sound and safe and fit for future use.
