Keeping you informed that I had tested COVID +ve on 16 th and 18th April’21 and I have undergone Home
Treatment. I’m currently collecting all the required documents for the Reimbursement/Claim. Due to
the Lockdown I’m not yet able to collect all the required documents for the claim, I will be submitting
the claim documents soon.

I had COVID-19, I have recovered now. Have taken the treatment at home, I’m having issues filling the
claim form as it is not suitable for home isolation and there is no separate form for Corona Kavach. Can
you please help me with the claims
