1.0 Purpose
This provides the procedure of installation CCB System scaffolding for Masinloc Power Plant
Expansion Project.

2.0 Scope
This procedure is applicable in accordance with the attached drawing fabrication intended for CCB
Scaffolding.

2.1 PROCEDURE

3.0 Roles and Responsibilities


3.1 VPCI Project Manager - shall be the overall responsible for the implementation of this
procedure ensuring that works are completed on schedule and in conformity with
drawings and specifications.

3.2 VPCI Project EHS Manager shall ensure safe implementation of this procedure promoting an
Incident and Injury Free environment.

3.3 VPCI & GTC Global Inc. QA/QC Engineer shall be responsible for the implementation
of construction specification and standards and see to it that this written method
statement of the work shall be carried out.

3.4 GTC Global Inc. Construction Manager – shall have the overall responsibility for the
direction, monitoring, coordination and control of all the construction activities attached
and related to concrete works and the overall Health and Safety procedure.

3.5 GTC Global Inc. HSE Supervisor along with the Safety Officer shall ensure
implementation of all safety procedures, safety measures related to the nature of works
being carried out and in accordance with the HSE Plan.

3.6 GTC Global Site Supervisor shall be directly involved in the planning and execution of
the activities. He shall be responsible for the preparation of work plan, safety action
plan and safety.

5.0 Procedure
5.1 PRE – INSTALLATION WORK

5.1.1 Scaffolds details shall follow the OSHA requirements.


5.1.2 Conduct pre–job orientation/briefing to all workers that would be involved in the
installation & erection of working platform
5.1.3 Secure all work permits accordingly prior to start of work.

5.1.4 Check all materials which are to be used conformance & in compliance to
specifications & integrity of the material and shall be maintained in safe
condition.

5.1.5 Scaffolds shall be erected in accordance with OSHA standard (OSHA 1910.28).

5.1.6 Each scaffold and scaffold components shall be capable of supporting without
failure.

5.1.7 Each platform on all working levels of scaffolds shall be fully planked or decked
and guard rail supports.

5.1.8 Start the work by bringing all material requirements on the area if possible on
the proposed nearest location of the working area.

5.1.9 Layout the location or boundaries of the proposed working scaffolding on site
and place safety barricades to ensure safe working area.

5.1.10 Lifting permit to be prepared prior to lifting if material lifting is necessary.

5.1.11 Lifting of scaffolding system materials required to second and third floor level.

5.2 INSTALLATION WORKS

5.2.1 For Center Columns (Combination of Tube & Coupler and System Support

Scaffold) 2 and 3 floor

5.2.1.1 Start the installation by erecting the 60.5 mm diameter post-pipe


system support for at least 350 mm away around the center columns
ensuring that the post is resting on base jack (refer to attached
drawing).

5.2.1.2 Install 3 levels of steel planks as working platform every 1.725 m lift
height resting on the horizontal tube and coupler and install phenolic
toe board of not less than 4 inches in height.

5.2.1.3 Install the diagonal bracing using tube and special swivel type coupler
connected to the vertical system support.

5.2.1.4 Install access ladder connecting to the working platform.

5.2.2 For Perimeter Column (Tube and coupler Scaffold) 2 and 3 floor

5.2.2.1 Start the installation by erecting the 48 mm diameter post-pipe system


support for at least 350 mm away at the 3 side of the columns on top
of the concrete slab ensuring that the post is resting on base jack.

5.2.2.2 Install steel planks as working platform every 1.900 m lift height at
about same level with the external scaffolds and install phenolic toe
board of not less than 4 inches in height.

5.2.2.3 Install the diagonal bracing and horizontal braces connecting the
external pipe scaffold to the column system scaffold using special
type of swivel and fixed connector.

5.2.2.4 Access to the column scaffold will come from the external pipe
scaffold system.

5.2.3 For External Scaffold (Tube & Coupler 20m Height) Ground to Roof Slab

5.2.3.1 Start the installation by erecting the 48 mm diameter post-pipe 400


mm away from the wall ensuring that the post is resting on base jack
with wood shoe plate and on flat & stable ground. The post-pipe
space at 1800mm using 6m and 4m pipe install alternate (refer to
attached drawing).

5.2.3.2 Connect with horizontal and diagonal braces members of the same
pipe diameter using fixed and swivel connector respectively (refer to
attached drawing details).

5.2.3.3 Ensure that all connections are properly secured. The 6m and 4m
pipe will be connected by round pipe connecting strands.

5.2.3.4 The runners shall be erected on a bay width of 1000mm located and
connected on both inside and outside post at even height. The bottom
runners shall be located as close to the base as possible.

5.2.3.5 The 6 m diagonal bracing will be fixed to every three horizontal pipes
in zigzag direction.

5.2.3.6 Vertical pipes and horizontal pipes will be connected by fixed clamp
while the diagonal pipes will be connected to stable vertical pipes by
swivel clamp.

5.2.3.7 Install steel planks as working platform every 1.900 m lift height with
phenolic toe board of not less than 4 inches in height.

5.2.3.8 Diagonal bracing will be installed connecting the inside scaffold at the

perimeter column of the 2 floor & 3 floor slab as out rig or counter
structure to secure the external scaffold and avoid overturning.

5.2.3.9 After concreting of the wall and column a free fabricated pipe welded
to the tie rod and connected to the embedded form tie rod with nut
and space at 5m maximum in horizontal & vertical direction. After

concreting of the 2 to 3 floor and 3 to roof perimeter column a
horizontal pipe clamp around the concrete column will be install and
serve as anchor of the external system scaffold (refer to attached
drawing).

5.2.3.10 The entire scaffold shall be tied to and securely braced against the
building at intervals not to exceed 30 feet horizontally and 26 feet
vertically as necessary (refer to (OSHA standard).

5.2.3.11 Guardrails shall be installed at all open sides of all scaffolds more
than 10 feet above the ground or floor.

5.2.3.12 Additional vertical pipes will also be installed every 3.65m if

necessary to re-inforce existing vertical pipes up to 2 floor level.

5.2.3.13 Stair access tower shall be provided in 3 locations (refer to the


attached drawing), also this will serve as outrigger and additional
outrigger will be install if necessary.

5.3 POST – INSTALLATION WORKS

5.3.1 Install life line for full body harness hooking during working if necessary.
5.3.2 Ensure that the structure is inspected & properly tagged (green tag) by certified
inspector prior to utilization.
5.3.3 Scaffold system shall be maintained and regularly check.
5.3.4 Dismantling of scaffold system, the erectors shall lower materials using a rope &
wheel or by the manual handling method known as the “handballing”. Whichever
method is used the erectors must at all times work from a minimum of a decked
working platform which has a handrail on all sides.

6.0 Environment, Health and Safety

6.1 All personnel must wear appropriate PPE at all times. Personnel are empowered to stop
work if safety precautions are not in place.

6.2 The Job Site Supervisor shall ensure that all Safety requirements are put in practice, copy of
MS and JHA (Attached) are available on site for guidance and consultation.

6.3 Contractor Safety Representative shall carry out the safety site inspection to ensure the
safety requirements laid out in the safe work plan are followed during and after the
completion of the works

6.4 Job Site Supervisor and Safety Representative shall be present at all time while works are in
progress.

6.5 Appropriate PTW to be prepared and implemented, modification without OWNER approval is
prohibited.

6.6 Assessed risk as necessary and communicated to concerned parties prior to start of any
work.

6.7 Safety briefing / Tool box talk must be conducted by Supervisors prior to start of work in the
language of workforce.

6.8 Incidents and injuries, damaged to property or near misses should be reported.

6.9 Good housekeeping to be maintained during the execution of activity.

6.10 A clear and good communication must be maintained by all personnel involved on the
activity.

6.11 Personnel must be trained/experienced and familiarized with the scope of works to be
undertaken. The location of marshalling point must be identified, emergency evacuation
procedure must be clear to all workers, exercises must be conducted.

6.12 Contractor Safety Manager / Representative shall review Safety Action Plan.
