SCHOOLS DIVISION OF TOLEDO CITY
GENERAL CLIMACO NATIONAL HIGH SCHOOL
Gen. Climaco, Toledo City
As stipulated in Division AIP in DLOAP, here are the objectives for this program:
• define learning goals and outcomes: identify specific topics or skills that will be taught to
students.
• determine how the learners will meet the learning goals by demonstrating knowledge and
• assess student knowledge and skill proficiency with the implementation of a learning
• collect, review, and analyze student learning outcomes assessment data to draw conclusions.
These are the activities that I have accomplished as a SLOAP coordinator in General Climaco
National High School for the School Year 2020-2021. First, I gave designation letters to three of my
co-teachers as SLOAP implementers who are Mrs. Mary Jane E. Tolero, Mr. Jhon Rey M. Balicoco
and Mrs. Noreen G. Semblante. Since the current situation in this pandemic seemed to be diverted
from the normal process and mainly driven by technology and social media, a need to convene and to
draft an action plan was the best thing to do. I gave letters of invitation to the three master teachers of
the school namely: Mrs. Ninque C.Solteral, Mr. Khen M. Villanueva and Mrs. Eva Mark Abrenilla with
the School Head Mrs. Victoria Nancy M. Nevado and Assistant Principal Mr. Kirk L. Capoarida. Their
expertise in this field is quintessential for the implementation of the SLOAP. I received the AIP-
DSLOAP and I drafted our School LOAP based from it and have our School Head approved it. The
meeting transpired last March 26 ,2021. We came up with an action plan for the whole year which
was to have a coordination meeting every quarter, gather the diagnostic test with TOS and answer
key, summative test with TOS and Answer key, Performance Task and Written Works, Class
Records, summary of competencies, consolidate and analyze the different evaluation and
assessment tools such as the Form 14 and item analysis. We have come up with a notion that we
shall reward teachers as best in SLOAP implementor to recognize their efforts of giving quality
assessments to our learners amidst this pandemic. Thus, we drafted the criteria and mechanics of
SLOAP best Teacher implementer. I made the different assessment tools (Self- evaluation form, Peer
Evaluation Form, Student’s evaluation form and Coordinators evaluation tool). The School Head gave
a memorandum to disseminate information for the plans that’s will be implemented throughout the
school year. On April 14, 2021, we held a LAC session in our school about the SLOA program. It was
held at the canteen in General Climaco National High School and almost all teachers attended except
for those who had other obligations attended. Every quarter, I gathered the summative test with TOS
and Answer key, Performance Task and Written Works, Class Records, summary of competencies. I
consolidated and analyzed the different evaluation and assessment tools such as the Form 14 and
item analysis. I submitted them to the SBM coordinator Mrs. Ziphorrah D. Bargamento, and SMEA
coordinator Mrs. Ninque C. Solteral , the School Head, Mrs. Victoria Nancy M. Nevado and to the
SLIP Coordinator Ms. Genivieve M. Templado. I gave a recommendation and a proposal for possible
intervention program (the Webquest and Chatbot Utilization). The results shall also be the basis for
the drafting of other intervention programs that will be beneficial both to the teachers and the learners.
