1. General

The objective of this testing procedure is to outline the berthing and mooring

provisions and determine if they are adequate for the intended vessel calling at

Cirebon 2 Coal Fired Power Plant Project. This report summarizes the jetty has

been constructed in accordance with the design criteria and local regulation.

In accordance with the design, the berthing speed is based on BS 6349-4 and

shall not berth the vessel during high wave condition. This testing will be carried

out on the wharf area in the Cirebon 2 Coal Fired Power Plant Project,

Indonesia with local government authority(KSOP).

4. Equipment and Measuring Instrument

1.1 Equipment

The berthing load test might use the vessel with a capacity of 13.,000 DWT.

The difficulties encountered in the implementation of testing to bring the vessel

in accordance with the criteria design. The vessel characteristic listed as

follows :

 LOA = 129.30 m

 LPP = 125.20 m

 D =8m

 B = 26 m

1.2 Measuring Instruments

For the measurement of displacement on berthing load test at wharf area, the

measuring instrument used is theodolite + camera + gauge.

5. Procedure for Recording Displacement

1.3 Prepare the recording displacement instrument on the piles at wharf area used
for loading test.on the pile loading test

1.4 Then, place the instrument in theat designated area.

1.5 The vessel which will be used for testing is preparedThe vessel which has
been decided for testing is prepared.

5.4 Perform the berthing of vessel at the defined area for testing (the berthing
angle, velocity, etc.)After that, berthed on the defined area as refer for the
testing ( the berthing angle, velocity, etc.).

5.5 In case of displacement occurred due to impact from vessel berthed, the
displacement will be recorded in the displacement recording instrument
(theodolite + camera)With the displacement happened due to impact when the
vessel berthed, the displacement will be recorded on the displacement
recording instrument (theodolite + camera).

5.6 Check if the displacement occurred within the design criteriaCheck the
displacement happened, whether it was in accordance with the design criteria.

5.7 Acceptence criteria displacement ≤ 100mm

7. HSE

7.1

7.1 PT Dharma Subur Satya and HDEC Field Engineer is responsible towards HSE Plan,

implement and managing issues regarding HSE concern during the work progress in this project.

7.2 PT Dharma Subur Satya and HDEC HSE Plan is prepared taking into account and following

Indonesian HSE Plan.

7.3 Safety Issue

Information to Personnel.

Before the commencement of work activities, ensure that all safety requirements are

provided.

Tool Box Meeting must be done and all personal assigned to do the task must understand

the hazards situation associated with the job and the control measures in place.

Safety induction must be given to all the personnel prior to entering the site and adequate

supervision by Safety Officer and Site Supervisor to ensure the job is carried out in the

safest way.

Adequate access and egress shall be provided.

Provide a safe and clean work place, proper tools, equipment and safe work procedures.

Insure that unsafe conditions which may contribute to injury, damaged or to operation

interruptions are promptly reported, corrected and action taken to prevent their
recurrence.

PT Dharma Subur Satya and PT HDEC Field Engineer has the direct responsibility for following

HSE Plan and managing issues regarding HSE for each works in this project.

7.2 PT Dharma Subur Satya and PT HDEC HSE Manager has the over-all responsibility for the

HSE implementation of in this procedure.

7.3 PT Dharma Subur Satya and PT HDEC HSE Plan and Indonesian HSE Plan shall be followed,

if for not mentioned HSE issue in this procedure.

7.4 Safety Issue
