INTRODUCTION:
We took universal company which works for the Engro fertilizer
company for the BOW TIE and SWISS model. Its work is mechanical in
which often they have to cut big pipes.

HAZARDS:

Grinder disc can break during the cutting cause pipes are so hard and
difficult to cut and the broken disc can hit any part of the body which
would make a huge loss.
While performing this job the employees often have to suffer through
severe and major injuries (face injury) and sometimes even death.

THREATS

 The grinder disc might get broken and hit the worker’s face which
would result in an obvious serious and severe injury.
 The disc may also hit his foot and result in severe injury.

TOP EVENT
The breaking of the grinder disc is the top event.

Consequences
 The worker or labor might get a severe injury.
 In case of face injury or some other major injury the worker dies.
PREVENTIVE AND RECOVERY BARRIERS OF THREATS
 The company provides the tools to maintain the grinder’s position
to keep it good for working without any problem.
 The company also provides the face shields and the helmet.
 The company provides the safety shoes.

 The worker is made to follow safety rules strictly.

 The worker is given a new disc when one is damaged.
 BARRIER ESCALATION FACTOR OF THREATS
If the above both barriers happen even if followed the safety rules then
we do refer to maintain a good distance from pipes.
