GUIDELINES

This appraisal will be represent your rating of the appraise actual performance on his/her present job. You are
encouraged to appraise and communicate with appraise when you are doing assessment. Circle the figure for

5.Execellent Highly effective and excel in performing the job/instruction well above the expectation.
