LETTER OF COMMITMENT

I, Mr./Ms. _______________________________________ recipient of the FFS On Production

of High Quality Inbred Rice, Seed Certification and Farm Mechanization Scholarship funded by
TESDA, and administered by Honorio Clemencia Integrated Farm, hereby commit myself to the
following.

1. I will complete all the module requirements and perform all the activities during the
RCEF Training Program;
2. I will maintain close contact with the registrar of Honorio Clemencia Integrated Farm
during and after the completion of the course. This will be done through Text Messages,
Phone calls, FB messengers and Emails.
3. When employed in lieu of the RCEF Training Program, I will furnish the Institution of
my employment certificate for documentation purposes;
4. I agree to Honorio Clemencia Integrated Farm visiting my farm following the completion
of the RCEF Training Program.

I understand that failure to comply with the aforementioned conditions will result to the
revocation of my SCHOLARSHIP and I have to refund the cost of scholarship amounting to
Fifteen Thousand Pesos Only ( P15,000.00) to Honorio Clemencia Integrated Farm.

I also understand that failure to complete the entire program and all its requirements will bar me
from availing any TESDA SCHOLARSHIP PROGRAM in the future.
