AFFIDAVIT OF DISCREPANCY

I, JOVINEL MALICDEM PAGULAYAN, of legal age, Filipino, married, and a

resident of Sisim, Penablanca, Cagayan, after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That I am the Biological father of twins PRINCE EVAN NANTES PAGULAYAN and
PRINCE IVAN NANTES PAGULAYAN, who was born on 17 JULY 2009, and the fact
of their birth was registered before the Local Civil Registrar of Penablanca,
Cagayan. Attached as Annexes “A-1 and A-2” are copies of Certificate of Live
Births;

2. That I and my wife, JUVYLYN SAYO NANTES got married last 11 SEPTEMBER
2011 before the Municipal Mayor of Penablanca, Cagayan. Attached is a copy
Certificate of Marriage as Annex “B”;

3. However, in the Certificate of Live Births of my twins, the name of my wife

appeared as JUVILYN SAYO NANTES-PAGULAYAN instead of JUVYLYN SAYO
NANTES-PAGULAYAN, and the date of our marriage appeared as 5 MAY 2008
instead of 11 SEPTEMBER 2011;

4. That I declare under the penalty of law that the names JUVYLYN SAYO NANTES-
PAGULAYAN and JUVILYN SAYO NANTES-PAGULAYAN refer to one and the
same person, and that is me, and that our date of marriage is 11 SEPTEMBER
2011 and not 5 MAY 2008;

5. That I am executing this affidavit to attest to the truthfulness of the foregoing

narration of facts and for any legal purpose.

IN WITNESS WHEREOF, I have hereunto affix my signature this 27 th day of
