SUBJECT: Report on fire incident and recommendations.

Last night at 4 am, a fire broke out at our organization’s ground floor storage room. Supervisor
Hasan Kamal as well as few workers had witnessed the incident. However, no casualties were
reported in the incident that took place.

Our inspection suggests that due to smoking cigarettes inside the storage room, the fire broke
out. However, no one was hurt as supervisor Hasan Kamal led a fire drill and everyone exited the
building. Nevertheless, the fire damaged 3 lakh taka worth of materials and shut down
production for the next three days.

There is also the problem of the security at the factory. Our company operates three 8- hour
shifts while having 2 supervisors. It is difficult for them to control or even to manage time order
as there are so many workers coming and leaving at all hours about which you were notified
earlier on 12th March 2021. There are many areas that are not properly lightened up at night times
as well as some doors are unguarded at times. In addition, the workers are not willing to listen to
the supervisors as they are both quite junior at the Factory. Moreover, some workers often take
cigarette smoking breaks inside the building since they do not have a break room. It is said that
the main cause of the fire that took place last night was because of smoking cigarettes inside the
storeroom.

Here are several recommendations to improve the situation at the factory. Firstly, more skilled
supervisors should be employed. Secondly, there should be proper lighting in the office also we
should hire security guards. Thirdly, there should be a common room for the workers so that they
can take cigarette smoking brakes between shifts without causing any hindrance in the factory.
Finally, we should ensure clean workspaces, daily checks of all machinery and safety and
security equipment and keeping clear access to always exits and first-aid stations.
