1. This refers to the request of the undersigned for transfer of assignment.

2. Relative thereto, the undersigned respectfully requests from your good office for
approval of Transfer of Unit Assignment from my current Assignment at Office of
the Provincial Fire Director BFP-R5 to Parañaque City Fire Station, Fire District
III, BFP-NCR for the following reasons, to wit:

a. That I wanted to grow beyond my comfort zone.

b. That I wanted to help my friend, Fiona to be reassigned in Camarines Sur

because of her health and family concerns.

c. That FO1 Ivy Fiona M Pili, 25 years old, currently assigned at

Parañaque City Fire Station, Fire District III, BFP-NCR, agreed to
exchange/ swapping of assignment.

3. In this regard, I am fervently hoping and wishing that this request be granted and
be given attention.

4. I humbly thank you Sir for making time to consider my request and await for your
decision.

Provincial Director for five (5) years and opts to continue performing her duties and
responsibilities of the Bureau in BFP-NCR.

2. That the undersigned is willing to help her friend, FO1 Ivy Fiona M Pili to be
reassigned in Camarines Sur because of health and family concerns.
