    1. Suppose you are to organize a NPO/CBO/NGO of any nature.

    a. Formulate your Vision, Mission and Goals.

    b. Design Programs/Projects using the Logic Model showing the specific goals, target
    beneficiaries, output, outcome and impact.

    *REMEMBER: Programs/Projects of any organization should be anchored from


    its VMG.

    c. Create a Monitoring and Evaluation Tools based from your Logic Model.

    *NOTE: Monitoring Tool is separate from Evaluation Tool. Monitoring Tool is


    used when the program/project is still on-going while Evaluation Tool is used
    upon completion of the program/project.

    2. Based from the Logic Model you designed for the programs/projects of your agency,
    create a Monitoring and Evaluation Tools that will measure their effectiveness, efficiency
    and sustainability.
