Health and Safety
If you have any health issues that may be of concern for an assessment, please speak to your instructor before attempting.
If at any stage, you become unwell during an assessment please inform the Instructor.
Your health is always more important, a re-assessment can be arranged.
Assessment Methods
Students will be assessed using various methods of assessment, from practical observations, theoretical knowledge, Case Studies &
Documented Discussion. Some questions may be multiple choice; some will require you to write your answer – where written
answers are required please write clearly and legibly.

All Assessments MUST be completed with a Black or Blue Pen – Students Do NOT use RED PEN,
(unless stated or required by regulatory requirements)

If at any point, you are unclear of the instructions for any assessment please ask your instructor or trainer for clarification and
explanation.

In a multiple-choice assessment if you wish to change your selected answer simply cross the Circle you originally placed and circle
the correct selection. See example provided.

Assessment Outcomes
If you have not satisfactorily completed either a Theoretical or Practical Assessment the instructor may request that you undertake
the complete exercise again, or ask you to repeat a portion of the exercise.

Theory assessments where a student has not answered correctly the Instructor may ask the question verbally or provide an
alternate question covering the same aspect of evidence. Both the Question Asked and Answer Provided MUST be documented
on this Student Assessment Pack next to the question being reassessed.

UNLESS STATED: ALL QUESTIONS MUST BE UNDERTAKEN AND CORRECT FOR COMPETENCY TO BE AWARDED.

Practical Assessments are limited to 2 additional reattempts, where a 2nd reattempt is required it cannot take place on the same
day as the original and 1st additional reattempt. If a student does not show satisfactory outcome to a practical assessment or a
proportion of a practical assessment after the completion of a 2nd reattempt the student will be graded as Not Yet Competent. The
student will not be awarded the unit of competency or where the unit forms part of a qualification, the qualification will not be
awarded. The scheduling of the 2nd reattempt will be at a time of agreement between MILCOM Institute and the student.
The 2nd reattempt cannot be on the same day as the initial and 1st reattempt.
Completion of Assessment
The student must complete the learner declaration on the last page to validate this assessment.
Outstanding Work
If a student has not submitted all required work for assessment before the end of the subject the instructor will nominate a ‘due
date’ for the work. If no due date is provided by the Instructor, then a standard 21 calendar days is implied. If a student fails to
submit outstanding work by the due date the unit may be graded as Not Yet Competent. Statements of Attainment, and where
these units, form/are part of a qualification, then the qualification cannot be issued, where units are marked as ‘Not Yet
Competent’.

Assessment Information

The assessment tasks for BSBWHS504 Manage WHS risks are included in this Student Assessment Tasks
booklet and outlined in the assessment plan below.
To be assessed as competent for this unit, you must complete all of the assessment tasks satisfactorily.

Assessment Plan
Assessment Task Overview
1. Written questions You must correctly answer all questions.
2. Prepare to manage WHS risks You are required to plan for managing WHS risks by
identifying applicable documentation and data to
determine the scope of hazard identification and
requirements to manage WHS risks. You will
document the scope requirements in a preparation
worksheet which includes a schedule of activities to
be completed in an appropriate sequence and
timeframe.
3. Conduct a risk assessment and implement You are required to implement your risk assessment
controls plan (developed in Assessment 2) and conduct a risk
assessment in accordance with legislative and
organisational requirements. Once complete, you
must develop and implement a fact sheet as part of
implementing risk treatments. The final part of this
task requires you to submit a screenshot of their
document storage.
4. Evaluate risk controls You are required to monitor the implementation of
the risk treatment plan and evaluate risk controls
based on data provided.
Assessment Preparation
Please read through this assessment thoroughly before beginning any tasks. Ask your assessor for
clarification if you have any questions at all.
When you have read and understood this unit’s assessment tasks, print out the Student Assessment
Agreement. Fill it out, sign it, and hand it to your assessor, who will countersign it and then keep it on file.
Keep a copy of all of your work, as the work submitted to your assessor will not be returned to you.

Assessment appeals
If you do not agree with an assessment decision, you can make an assessment appeal as per your RTO’s
assessment appeals process.
You have the right to appeal the outcome of assessment decisions if they feel they have been dealt with
unfairly or have other appropriate grounds for an appeal.

Assessment Instructions
Each assessment task in this booklet consists of the following:
Assessment Task Cover Sheet
This must be filled out, signed and submitted together with your assessment responses.
If you are submitting hardcopy, the Cover Sheet should be the first page of each task’s submission.
If you are submitting electronically, print out the cover sheet, fill it out and sign it, then scan this and
submit the file.
The Assessment Task Cover Sheet will be returned to you with the outcome of the assessment, which will
be satisfactory (S) or unsatisfactory (U). If your work has been assessed as being not satisfactory, your
assessor will include written feedback in the Assessment Task Cover Sheet giving reasons why. Your
assessor will also discuss this verbally with you and provide advice on re-assessment opportunities as per
your RTO’s re-assessment policy.
Depending on the task, this may include
• resubmitting incorrect answers to questions (such as short answer questions and case studies)
• resubmitting part or all of a project, depending on how the error impacts on the total outcome
of the task
• redoing a role play after being provided with appropriate feedback about your performance
• being observed a second (or third time) undertaking any tasks/activities that were not
satisfactorily completed the first time, after being provided with appropriate feedback.

Assessment Task 1: Written Questions

Task summary
This is an open book test, to be completed in the classroom.
A time limit of 2 hours to answer the questions is provided.
You need to answer all of the written questions correctly.
Your answers must be word processed and sent to the assessor as an email attachment.

Written answer question guidance
The following written questions use a range of “instructional words” such as “identify” or “explain”, which
tell you how you should answer the question. Use the definitions below to assist you to provide the type of
response expected.

Analyse – when a question asks you to analyse something, you should do so in detail, and identify
important points and key features. Generally, you are expected to write a response one or two paragraphs

Compare – when a question asks you to compare something, you will need to show how two or more

in a list format.

Task summary

As we have previously discussed, it is imperative that regular workplace inspections are being conducted. At some
stage this week I would like you to meet with your team to conduct a risk assessment of the office area in accordance
with our policies and procedures. It is important that all our staff have the knowledge and skills to perform risk
assessments and that they feel comfortable contributing to the hazard identification process.
I know you are still getting used to our policies and procedures, so I would like you to take some time today to read
through our internal WHS and risk processes to ensure you understand our requirements. Additionally, I would
suggest that you conduct some research online to identify data relating to workplace injuries and incidents (some
facts and statistics would be ideal) so you can clearly emphasise the importance of risk management to the team.
Your meeting preparation should include:
1. Overview of Grow Management Consultants policies, procedures and systems applicable to hazard and risk
management
2. WHS Acts, regulations, codes of practice, standards and guidance material and other publications relevant to WHS

Complete the following activities:

1. Conduct a risk assessment


With your meeting preparation complete, you must now meet with the team and take the lead
in conducting a risk assessment in your training environment. Your assessor will set up the
room to ensure there will be at least two hazards for identification. To do this, you will need to:
• Print a copy of the Workplace Safety Checklist (or have access to this on a handheld device)
• Meet with your team and discuss:
o Context of the risk assessment
o Process you will take
o Overview of hazards/risks to look out for
• Conduct a walk-around of your training environment and complete each section of the
Workplace Safety Checklist with your team. Everyone must contribute equally.
• During the meeting, you must provide the members of your team the opportunity to ask
questions.
• During and after the risk assessment you will be required to demonstrate a range of skills
including:
o Speaking clearly and concisely
o Using non-verbal communication to assist with understanding
o Asking questions to identify required information
o Responding to questions as required
o Using active listening techniques to confirm understanding
o Monitoring adherence to legislative and organisational requirements
• Save your completed document in your desktop folder (or upload photographic evidence if
handwritten)
• Submit to your assessor

2. Complete risk register

Based on the hazards you identified during the inspection you completed in question 1, you
must now determine the risks and applicable controls. To do this, you will need to:
• Based on the hazards identified in the Workplace Safety Checklist, document corresponding
risks into the Risk Register (a minimum of three risks must be documented)
• Using the risk rating in the Risk Policies and Procedures, determine the consequences,
likelihood and priority for each risk
• Evaluate and select a minimum of two (2) risk controls for each risk
• Document the person responsible, a timeframe and monitoring requirements for each risk
• Save your completed risk register in your desktop folder
• Send the risk register via email to the Operations Manager (your assessor) which includes
an overview of how the meeting went and a brief overview of the contents of your risk
register. The text of the email should be in grammatically correct English, written in an
appropriate (polite, business-like) style.

3. Implement controls

As instructed in the email from your manager, you must develop an ergonomics fact sheet and
send to all staff. To do this, you will need to:
• Develop a one-page fact sheet (using the Fact Sheet Template or by designing your own) which
explains:
o Ergonomic hazards and risks
o Ergonomic tips for correct workstation setup
o Three (3) ways to prevent musculoskeletal disorders
• Save your completed fact sheet in your desktop folder
• Send the fact sheet via email to all staff (your assessor) which includes an overview of
ergonomic hazards and risks and how they can be prevented. The text of the email should be
in grammatically correct English, written in an appropriate (polite, business-like) style.

4. Store documents
As instructed by your manager in Assessment 2, you are required to systematically store all of
your documents in your ‘Risk Management_Admin’ desktop folder. You must now provide
evidence of this. To do this, you will need to:
• Take a screen shot of your desktop folder
• Paste it into a blank word document or save as a JPEG file
• Submit a copy to your assessor
