INTERNSHIP AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Internship Agreement entered into by and between SHIRLY PEÑONES of legal age and
with postal address at ML. QUEZON ST. SAN ROQUE, BUHI, CAMARINES SUR, herein referred to
as the STUDENT INTERN, and JOSEFIN N. OLIVEROS of legal age iand with postal address at
SAGRADA BUHI, CAMARINES SUR, herein referred to as the FACULTY INTERNSHIP
COORDINATOR, and CAMARINES SUR POLYTECHNIC COLLEGE BUHI CAMPUS a legal entity
duly organized and existing under Philippine laws, with principal address at SAN BUENA, BUHI,
CAMARINES SUR, represented by _____________________________, herein referred to as Host
Training Establishment (HTE), ______________________________________ of legal age and with
postal address at ________________________________________________, herein referred to as On-
site Supervisors.

Witnesseth:

Whereas, the College of BUSINESS MANAGEMENT of CSPC offers a ladderized program


leading to the degree/diploma in Bachelor of science in office administration. As a requirement for
graduation, students should undergo internship for a minimum of 200 hours to expose them to actual
work along their field of specialization, thereby augmenting their formal schooling.

Whereas, _________________________________________ entered a memorandum of


agreement with CSPC to forge an industry academe linkage program to assist in developing the
manpower resource of the region, in particular, and of the country in general.

NOW THEREFORE, for and in consideration of the foregoing premises the parties herein agree
and bind themselves to extend full support and allow to use its facilities and render professional services
to student intern under the terms and conditions herein stipulated.

A. Duration of Internship

1. That the internship program is scheduled to run for a period of 200 hours or equivalent to
_____________ weeks which shall start on ______________________ and shall end on
___________________ ;

2. That the internship hours should not exceed the maximum number of hours of work prescribed by
law, if any, for a worker of his/her age and sex. Time spent in related theoretical instructions shall
be considered as internship hours and shall be reckoned jointly with internship training time in
computing the required internship period.
B. Obligations and Responsibilities of Parties

1. That the Student Intern Shall:


a. Submit to the training host the following before the start of the internship:
1. Personal data sheet;
2. Parent’s Consent/Waiver form;
3. Approved application for student internship program;
4. Proof of life/accident insurance coverage;
5. Medical certificate issued by the College Physician or any government physician
attending to the fitness to undertake the internship assignments; and
6. Duly notarized Internship Agreement.
b. Observe the existing office decorum and policies of the training host (e.g. punctuality,
attendance, working hours, official uniform) for the duration of internship;
c. Perform diligently the internship assignments given to him/her;
d. Pursue faithfully the prescribed course of study and take advantage of the opportunity to
improve his/her efficiency, knowledge and personal traits so that s/he may enter chosen
occupation as a desirable employee in the future;
e. Not engage in gambling, illicit activities, drinking intoxicating beverages and related activities
while at work or within premises of host training establishment;
f. Shoulder all expenses, including travel and transportation expenses for the duration of the
internship program;
g. Accomplish weekly internship journal that will be periodically checked by the designated on-
site supervisor/mentor;
h. Inform the on-site supervisor in advance if s/he is not reporting to the office, and make up for
days absent to complete the required number of days/hours of training;
i. Be liable for any breakage and/or destruction of any equipment or laboratory facilities resulting
from his/her own doing;
j. Submit required reports/documents at the end of the internship period.

3. That the Faculty Internship Coordinator shall:


a. Be responsible for all aspects of the student internship program including program
implementation, monitoring and evaluation;
b. Be expected to conduct an initial site visit to ensure that the training facility is safe and
conducive for the student intern;
c. Be expected to review, orient, interpret and clarify to the student intern the objectives of the
internship program;
d. Do a regular monitoring of the student intern under him/her to check on the overall
performance and discuss with the on-site supervisor to further improve the internship program;
e. Meet with student intern on a periodic schedule for student feedback on his/her assignment to
validate complaints of both parties, if any;
f. Be available for consultation with student intern and provide coaching and counselling
assistance, if needed;
g. Be responsible in evaluating the student intern’s reports, self-evaluation and give the final grade
taking into consideration the evaluation of the on-site supervisor.
4. That the Host Training Establishment (HTE) shall:
a. Conduct an orientation for the student interns detailing the intern’s duties and responsibilities
during the duration of the internship;
b. Assist the designated Faculty Internship Coordinator in monitoring the active participation and
attendance of student intern;
c. Not be liable for any accident or body injuries occurring to the student intern and for any loss of
a student intern’s personal property while in the premises of the training host for the duration of
internship;
d. Be responsible in providing actual internship to the student intern by assigning him/her to any
department during the duration of the internship requiring for exposure to actual work;
e. Clearly explain work assignments to ensure that the duties, role and responsibilities of the
student intern are clearly defined and outlined;
f. Assign a focal person or on-site supervisor who will monitor the student intern’s over all work
performance including demeanor, competence and professionalism;
g. Conduct an evaluation of the student intern’s overall performance based on the agreed
standards or requirement with the school;
h. Issue a Certificate of Completion to the student intern upon completion of the internship
program.

5. That the On-site Supervisor of the HTE shall:


a. Be responsible in providing the student intern with opportunities to work with the specific
department or area where he/she can best apply the theories, principles and ideas learned in the
academe;
b. Be responsible in coordinating with the different departments of the training host where student
intern will report;
c. Be responsible in supervising the student intern for the duration of the internship program;
d. Facilitate the evaluation of the student intern’s performance using the agreed upon standard or
requirement of the College;
e. Regularly meet with the Faculty Internship Coordinator to discuss his/her evaluation
observations and other issues that may arise for the duration of the internship program
f. Prepare the evaluation of the student intern, upon completion of the internship program and
submit such evaluation to the Faculty Internship Coordinator.

C. Other terms of Conditions

1. That while on the process of internship, the student shall have the status of student intern and
shall not be deemed as displacing a regular worker nor substituting for a worker needed by the
training host;
2. That the Faculty Internship Coordinator shall have the authority to transfer or withdraw the
student intern anytime with proper protocol;
3. That, if student intern wants to transfer, he/she should file written request and clearance from
the training host and present it to the on-site supervisor for proper and official endorsement;
4. That the student intern can refuse to perform task without fear of reprisal, when the student
intern reasonably believes that the tasks will endanger him/her or another person. It is the
parties’ responsibility to assure that working conditions are safe before starting any task;
5. That, if the student intern develops methods or inventions during the internship that are
considered a direct result of the cooperation with the Training Host and if of its interest, the
latter is entitled to the methods and inventions;
6. That, if this Internship Agreement has failed to be fulfilled, all involved parties can annul the
agreement immediately. Termination of this agreement has to be in writing to the other parties.
The resigning party has to inform the concerned Dean in writing.

IN WITNESS whereof, the parties have executed this Internship Agreement this ___th of
