A RESOLUTION AUTHORIZING THE MUNICIPAL MAYOR HON. ALICIA RAMA-

TY, FOR AND ON BEHALF OF THE MUNICIPALITY OF PLARIDEL, PROVINCE
OF MISAMIS OCCIDENTAL, TO SIGN THE AMENDED MEMORANDUM OF
AGREEMENT (MOA) WITH CLARENZON MARKETING RELATIVE TO THE
PROVISION OF A BRAND NEW AUDICOM ELECTROLYTE FULLY AUTOMATED
ANALYZER FOR THE EXCLUSIVE USE OF THE PLARIDEL COMMUNITY
HOSPITAL (PCH) UNDER A TIE-UP PLAN/AGREEMENT

WHEREAS, it is the policy and priority of the Local Government Unit of Plaridel
to provide Plaridelnons with quality health care and effective medical services at the
Plaridel Community Hospital;

WHEREAS, the LGU recognizes the need to advance and promote the
modernization of its health facilities to effectively address the health concerns and
medical needs of its constituents;

WHEREAS, it was requested to this Honorable Body to bind the Local

Government Unit of Plaridel in a Memorandum of Agreement with Clarenzon Marketing.
The agreement pertains to the provision of laboratory equipment to the Plaridel
Community Hospital (PCH) under a tie-up plan, attached therewith is the amended draft
MOA;

WHEREAS, Clarenzon Marketing (Clarenzon for brevity), a company duly

organized and existing under Philippine laws, is a private entity engaged in the business
of providing laboratory equipment and accessories, and medical supplies;

WHEREAS, Clarenzon and the Local Government Unit has agreed that Plaridel
Community Hospital laboratory shall have a five (5) year equipment reagent tie-up
under a direct contracting plan agreement, since Plaridel Community Hospital has no
existing Electrolyte Analyzer and Immunology Analyzer as their preparation to meet the
demands during licensing seminars and Department of Health inspection as per
requirement.

NOW THEREFORE, on motion of SB Member Rojim R. Banguis, duly seconded

by SB Member Algelyn B. Paradiang and SB Member Lucita M. Aviles, with the favorable
support of all the SB Members present, it was –

RESOLVED as it is hereby RESOLVED to AUTHORIZE THE MUNICIPAL

MAYOR HON. ALICIA RAMA-TY, FOR AND ON BEHALF OF THE MUNICIPALITY
OF PLARIDEL, PROVINCE OF MISAMIS OCCIDENTAL, TO SIGN THE AMENDED
MEMORANDUM OF AGREEMENT (MOA) WITH CLARENZON MARKETING
RELATIVE TO THE PROVISION OF A BRAND NEW AUDICOM ELECTROLYTE
FULLY AUTOMATED ANALYZER FOR THE EXCLUSIVE USE OF THE PLARIDEL
COMMUNITY HOSPITAL (PCH) UNDER A TIE-UP PLAN/AGREEMENT

RESOLVED FINALLY, that a certified copy of this Resolution be furnished to

the Municipal Mayor Alicia Rama-Ty, Clarenzon Marketing, the Plaridel Community
Hospital and other offices, departments and agencies concerned for their information,
guidance and reference.
