SUB: Request for consideration of a resolution plan in terms of the Circular issued by the Reserve Bank
of India (“RBI”) bearing no. RBI/2021-22/31 DOR.STR.REC.11/21.04.048/2021-22, dated May
5, 2021 (“Circular”).

Dear Madam/Sir,

This is with reference to the loan availed by me/us vide the Loan Account Number as mentioned above.

I/ We would like to inform you that I am/ we are facing financial stress due to the outbreak of the Covid-
19, which has impacted our/ my repayment capacity as under:

- Involuntary loss of employment, being my only source of income, arising out of lay-offs undertaken
by my employer after the outbreak of the Covid-19;
- My/our business shut down temporarily/seriously hampered after the outbreak of Covid-19, due to
lockdown restrictions;
- Significantly reduced family income after the outbreak of the Covid-19 owing to lockdown restrictions.

In this regard, I am aware that RBI has issued the Circular dated May 05, 2021 enabling lenders to
implement a resolution plan in respect of retail loans. In view of the above, I/ we have decided to approach
you to request for considering a resolution plan with respect to my/our loan availed by me/us vide the Loan
Account Number as mentioned above. I/We further state that I/we have not availed of any Resolution Plan
under the earlier RBI Circular dated August 06, 2020, other than seeking for moratorium, as may be
applicable.

I/We request HDFC to kindly consider a resolution plan, inter alia as HDFC may deem fit (“Resolution
Plan”).

I/We further agree and undertake, that invocation of the resolution and implementation of the Resolution
Plan in respect of my/ our loan account shall be at the sole discretion of HDFC and shall be subject to the
terms and conditions of the Circular. HDFC shall have the right to assess my / our request and further shall
have the right to not consider the request for resolution for such reasons as HDFC may deem fit in its sole
discretion. I/ We further agree and confirm that the date of implementation of the Resolution Plan shall be
as stated in the agreement to be entered into in connection with the Resolution Plan.

I/We further agree that in the event HDFC accepts my/our request for invocation of the resolution and
implementation of the Resolution Plan, the same shall be based on considerations and assessment of
viability by HDFC of certain parameters of eligibility as represented by me/us to HDFC. In the event any
change in such parameters or misrepresentation by me/us, HDFC shall, apart from any other right that it
may have under the Loan Agreement/ Facility Agreement or under applicable law, inter alia, have the
option to reject the request for invocation of the resolution and/ or implementation of the Resolution Plan,
and any in-principle approval granted to me/ us in this regard shall stand cancelled forthwith without any
act on the part of HDFC.

I/ We further agree and undertake to forthwith execute all documents and provide all information as may
be required by HDFC in connection with the Resolution Plan, including for the purposes of implementation
of the same.
