AUTHORIZATION LETTER

KOLIEN ARCULAR CUBILLAN, to do the transaction in updating my SSS account with the SSS
forms I have filled out, as I cannot personally visit your office for I am currently pregnant. He is
further authorize to sign, in my behalf, any documents that is necessary in furtherance of this
authorization.
Attached are the copies of my Voter’s ID with the number 6723-0007B-J1994KGS20000, my
Certificate of Live Birth, my Certificate of Marriage, and the Certificate of Live Birth of my child.
Please also note that my signature in my Voter’s ID and Certificate of Marriage may differ from
that of my signature in this letter and in the SSS forms, as I have changed my signature after I
got married.
