Depression

Situation: Lucy has been experiencing low mood and symptoms of depression. She described
these to her GP, who referred her for psychological therapy. This is her first session.

1. Assessment
a. Depression mhGAP master chart assessment
i. For at least 2 weeks, has the person had at least 2 of the following core
depression symptoms: (applicable to the client)
1. Px has been experiencing low mood.
2. Px has low interest or pleasure in doing activities.
3. Px feeling down, sad & no motivation.
4. Px lived with her friends but she can’t talk to them.
i. During the last 2 weeks has the person had a least 3 other features of
depression: (applicable to the client)
a. Ideas of guilt and unworthiness
b. Px reduced concentration and attention in school
c. Px reduced self-esteem and self-confidence for herself.
ii. Does the person have difficulties carrying out usual work, school,
domestic, or social activities? Yes
iii. Does the person have bipolar disorders? No
iv. Does the person have depression with psychotic features (delusions,
hallucinations, stupor)? No
v. Concurrent conditions? None
vi. Person is female of childbearing age N/A
vii. Person is a child or an adolescent? Yes
2. Intervention
a. Cognitive Behavioural Therapy (CBT) – based on the idea that feelings are
affected by thinking and behaviour. CBT typically has a cognitive component
(helping the person develop the ability to identify and challenge unrealistic
negative thoughts) and a behavioural component.
• Identifying troubling situations or conditions
• Become aware of own thoughts, emotions, and beliefs about these problems.
• Identify negative or inaccurate thinking. 12:40
• Reshape negative or inaccurate thinking.
b. Family counselling or therapy - Family counselling or therapy should include the
person if feasible. It entails multiple (usually more than six) planned sessions
over a period of months. It should be delivered to individual families or groups of
families. It has supportive and educational or treatment functions. It often

includes negotiated problem-solving or crisis management work. This


therapeutic intervention will make recognize their relationship as a family since
according to the patient, her family is experiencing some marital conflict. By this
intervention, they can express their feelings and perceptions to one another.
Hence, their daughter or the patient’s perspective about their current situation.
