RESEARCH SUBJECT INFORMED CONSENT FORM

To the Prospective Research Subject: Read this consent form carefully and ask as many questions
before you decide whether you want to participate in this research study or not. You are free to at any
time before, during, or after your participate in this research

1. PURPOSE OF THIS RESEARCH STUDY


The goal of the study is to spread awareness, knowledge, and information about the
controversial issue and to enlighten people about the impacts of police violence in the society. The
objective of the researchers is to encourage the oppressed individuals by political power and gun
power in such circumstances to ensure that they would be heard. It is included that the influence of
people's trust in law enforcers and community’s sense of security are continuously decreasing.

2. PROCEDURES
The purpose of interviewing the teenagers, millennials, and seniors to know on what their
perspective in police violence common issues or problems these days are and to know some more
information on how they are aware of some news that they watched or heard. Specifically, you will be
asked to the following:

 You will be interviewed at least for an hour about your opinions or perspectives about police
violence.
 You will be asked to confirm the transcript of the interview.
 You will be asked to evaluate your own answers or opinions as the participant to this interview.

The expected duration of your participation which will be done intermittently is from April until
st
the 1 week of May.
3. POSSIBLE RISKS OR DISCOMFORT

The possible risks or discomforts of the study is minimal. You may feel a little uncomfortable,
embarrassed, and sad. Answering it personally, sensitive, and many survey questions. However, there
is no possible time consuming in your part.

4. POSSIBLE BENEFITS

This research will help everyone and other researchers spread awareness about the police
violence happening in our country. It may also be the way to reduce the police violence cases in the
Philippines.

This study will also be helpful for future researchers that are interested in this kind of topic and
improve the knowledge of the students, workers, and others about the police violence in the
Philippines.

5. FINANCIAL CONSIDERATIONS

When it comes to finances, the researchers did not encounter any contrary quandaries.
Because of the inconsistent changes of the community quarantine restrictions, the researchers could
not operate outside to conduct a field study and print out hard copies of the paper. However, a stable
internet connection will be needed for the data instruments to be performed. Approximately one
thousand five hundred to two thousand pesos of internet bill per month (₱1,500-₱2,000) is
necessitated for the researchers to complete the whole process of the study.

6. CONFIDENTIALITY

The researchers will prevents the unauthorized use or disclosure of information, ensuring that
only those who are authorized to access information can do so. Base in the law to preserve ethical
standards and the ethics of the research process, to protect the privacy of all individuals, to establish
trust and rapport with study participants, and to maintain ethical standards and the integrity of the
research process, researchers will preserve the answers of the interviewees as anonymous. All of the
interviewee's response on the data collection will remain anonymous as the researchers publish their
study.
7. TERMINATION OF RESEARCH STUDY

All of the chosen participants of this study have the rights to not participate unless they really
don’t want to. You are not oblige to participate if you’re really not into interviewing or not sociable.
Every chosen participants have the rights to choose on their own decisions and the researchers will
truly appreciate it and respect it. The participation has no payment needed.

9. AUTHORIZATION
I have read and understood this consent form, and I volunteer to participate in this research
study. I understand that I will receive a copy of this form. I voluntarily choose to participate, but
I understand that my consent does not take away any legal rights in the case of negligence or
other legal fault of anyone who is involved in this study. I further understand that nothing in this
consent form is intended to replace applicable laws.
