This article describes the salient features of the McGill model of nursing, (ie,
health, family,
collaboration, and learning), within the health, person, environment, and nursing
paradigm.
According to the model, the central goal of nursing is to maintain,
strengthen, and
develop the patient's health by actively engaging him or her in a learning
process. Because
health is a learned phenomenon and the family is considered the primary
socializer in this
learning, the family is the focus of nursing. The nurse strives to
structure a
learning environment that enables the patient to participate as fully as
possible. The
nurse and patien£ together set goals and, building on the patient's
strengths and
resources, devise means of achieving them.
THE MCGILL MODEL of nursing was H develope‹1 over many years by the
faculty and
students of McGill University School of Nursing under the guidance and nurturance
of Dr Moyra
Allen. During the past decade, elements of the model have been identified,
tested, and
elaborated in practice and through discussion."'2 Al- though great
progress has been
made in developing this model, much work still remains. This article
presents the work
to date on the model. The salient features of the model will be examined
within the health,
person, environment, and nursing paradigm. This paradigm has received
widespread
acceptance within the nursing profession as a useful approach to examin- ing
and subsequently
evaluating a mod- el."’" Before proceeding with an examina- tion of the model,
a brief
historical note on the model's origins and the approaches taken in its
development are in
order.
The impetus for the development of the McGill model of nursing can be traced
to a complex
interplay among societal, con- sumer, and professional forces that trans-
pired at about
the same time and crystal- lized the key elements of the McGill mod- el.""
The 1960s
heralded a new approach to the delivery Of health care services in
Canada. This approach
subsequently led to a reconsideration of the role of the nurse within the
health cabe
system. With the introduction of a universal health insur- ance plan
predicated on
the conviction that health services be available, accessible, and responsive
to the needs
of all Cana- dians, many health care workers carne to believe that
nursing was the
profession to meet this challenge. Because they repre- sented the largest
work force in
the health care system, nurses were seen as the group to supply the additional
labor needs
within a reasonable cost structure. However, debate within the
nursing profession
arose as to how best to meet this challenge.
One position perceived improved health care as synonymous with improved
medi- cal care.
Within this perspective, nursing's role would be one of replacing physicians,
ie,
performing medical tasks previously assumed by physicians. Buoyed by
the trends
in the United States and investiga- tions carried out at McMaster
University," health care
leaders established nurse practi- tioner programs in Canada. However, not all
Canadian
nursing leaders supported this new direction.
A second view for the role of nursing was espoused, with Dr Moyra Allen
as its major
proponent. She observed that the
direction taken in the first view, rather than
expanding and developing nursing's role as an independent health care profession,
was in effect
replacing the functions of another profession. Furthermore, evidence was
mounting that
demonstrated that the tradi- tional medical approaches to major ill-
nesses had little
effect on morbidity and mortality rates.¹' Many of these health
problems, it was
noted, were linked to life styles and established health habits learned within
a family
context.ᵗ⁷," Thus, advo- cates of this second position argued that
providing more of
the same types of ser- vices was not the solucion. Rather, nurses were
needed as
primary promoters and facilitators of family health. Consumers were
also advocating
for an increase in family health services and showed signs of readiness
to assume
greater responsibility for their health and become increasingly involved in
healthier life
styles."’"
As these events were unfolding, the faculty at McGill University had
already begun
to emphasize the practice of nursing as a unity. Nursing situations
involving clients of
varying ages and diagnoses in different settings were analyzed using
a groonded
theory approach.²⁰ From this analysis, the critical elements of the model
began to
emerge. Work then began on identifying the elements that distinguished
the McGill
framework of nursing from others.
Once the rudimentary elements of the model had been identified, it was
recog- nized
that further refinement, description, and elaboration could take place
only within
nursing practice. Settings outside the existing health care system
needed to be created
in order to facilitate experimen- tation with the model. These settings,
known as
demonstrations (funded by
Health and Welfare Canada, projects No.
605-1234-46 and 605-1300-42), were the
laboratories wherein the elements of the model were tested,
clarified, refined,
reshaped, and further developed. Some of the demonstrations had a btiilt-in
research
component for monitoring, describing, and evaluating the development of
nursing practice
and hence the model of nurs- ing.""' Like many models, this model
had its origins
in a particular setting, in this case, the community. However, the McGill
model was and
is intended to be a broad- based model.

From this premise follows a second major assumption, that is, that
individuals,
families, and communities do indeed aspire to and are motivated toward better
health.
Furthermore, there resides within these persons the potential to
develop and
achieve this end. A third assumption is that health is best learned
through active

Certain characteristics of the person fol- low from this systems orientation.
First of all,
humans are evolving and developing over their life span. At the same
time, the
individual's or family's behaviors are char- acterized by continuity and
stability, that
is, they are not isolated or idiosyncratic acts but rather are part of
larger patterns of
behavior. Moreover, these behaviors are goal directed, which implies that
they have purpose
and meaning.
The "person" in the model is active and is a problem solver, capable of
learning new
ways of coping with situations and problematic events. Furthermore,
each experience
serves a "training function. ²⁷ That is, the individual or family
learns
something in the process of dealing with such experiences. The person
within the
McGill model, therefore, is capable of learning from previous
experiences and
building on these in order to achieve those goals deemed important. Inherent
in the model is
the belief that the solutions to the problems or situations confronting the
individual or
family reside within the fam- ily and do not emanate from the nurse.
Therefore,
rather than passively receiving the nurse's ministrations, the individual or
family
collaborates with the nurse to work toward a common goal of healthier living.
Indeed, the model
claims that in order for change to occur, the person (individual or family)
must become
an active participant in his or her care. Furthermore, the likeli- hood
of active
participation is increased when the goals are those of the client.
Such views presuppose that the person is motivated to assume responsibility for
his or her
health and that there resides within the individual or family the
strengths,
resources, and energy necessary to do so.
The model recognizes that these potentials will vary from individual to individual
and from family
to family. Each person brings to the situation a different genetic endow- ment,
personal
history, and physiological, psychological, and social makeup.
Environment
Surveying extant nursing models, one becomes aware of the varying
interpreta- tions
of the environment. In the McGill model, the environment is viewed as
the context
within which health and healthy ways of living are learned. Although the
family is
considered the prime context within which health is learned, it is
recog- nized that
learning about health does occur in many varied settings such as acute care
units, community
health centers, long-term care institutions, schools, and work
places."
The person and the environment are not viewed in isolation from one another
but rather in
constant interaction, each influencing and being influenced by
the other. As
a result of this interchange between person and environment, learning
occurs. Por
example, viewing the family as a context for learning healthy ways of
coping, the
question might be asked, "What forces are operating in a particular
family that are
resulting in a child's ability (or inability) to cope with entering school
in order to assist the individual or family in learning new ways of dealing with a
situa- tion or
achieving healthy ways of family functioning.
Nursing
According to the McGill model, the primary goal of nursing is health
promo- tion.
Since health is a learned process, the goal of nursing is to engage the
individual or
family in the process of learning about and acquiring healthier ways
of living.
Learning about health is thus contingent on the nurse's structuring
learning experi-
ences that are directed toward meeting the needs, goals, and problem-solving
styles of the
individual or family. Changing or strengthening individual or family
behav- iors
is also predicated on utilizing the individual's or family's
potentials and
resources. To profit from such experiences, the individual or family must
become an active
participant and collaborator in the health promotion process.
Negotiation,
collaboration, and coordination are there- fore features of the nurse-patient
relation- ship.²
These features are present during the assessment, implementation, and
evalua- tion phases
as the patient and nurse work together on a variety of health-
related situations.'

Assessment phase
The aim in assessment is for the nurse to obtain an overall pro6le and
understanding of the
health aspects of the person. The nurse also gathers information on
the nature of
the environment in order to identify its eífects on learning
about
health. The patient is the major provider of information from which nursing
operates.
Furthermore, the patient serves as the prime validator for the
nursing interpreta-
tions.
Even though the individual or family may present with a specific
concern or
request for assistance, the nurse attempts to place this concern within a
health con-
text. The nurse looks for general patterns of behaviors or approaches
to living by
searching for variations and similarities across diverse situations.
The nurse
also gathers information on the client as a learned. Readiness to
engage in
health work, desired goals, level of motivation, interest, energy, and modes
of learning
are ascertained from the client. In addition, the ind ivid u al's or
family's potentials,
strengths, and ways of using both internal and external resources are
identified. With
this information the nurse begins to gain some ideas about how to
structure a learn- ing
environment that will stimulate, cap- ture interest, and elicit the
client's partici-
pation and involvement. Moreover, the information enables the nurse to
consider ways
of nursing that are congruent with and tailored to the client's
goals, needs,
interests, and approaches to learning.
The assessment phase reqtiires an explor- atory approach to the gathering of
infor- mation,
which contrasts with entering a situation with a prescribed or
predeter-
mined plan of action. However, it is impor- tant to note that along with
information obtained
from the client, the nurse uses other sources of information, knowledge,
and experience
to rationalize his or her observations. As Stevens comments, pa- tient
and nurse come
together in a ”part- nership of unequals, 'unequals' in that each has a separate
and different
expertise from the other. ²" "" Since health is dynamic and evolving, the
nurse must also
continu- ously revise, modify, update, and validate the client's profile.
The assessment phase also sets the stage for the respective roles to be played
by the nurse
and the client. For the client to assume an active role in his or her
care, the nurse
should create a supportive environ- ment in which the client feels
safe and valued.
By acknowledging the client's cen- tral role in the health-promoting process
and by
conveying acceptance, the nurse sets the tone for future involvement and
participation.
Even though the client's database may be incomplete, the nurse and client
begin a
process of negotiating and testing diíferent strategies for working on
health matters.
This phase of the process, as with other phases, is subject to change as
learning needs
become apparent. During this phase, mutual, personal, social, and
situational
understandings are achieved, some general direction is agreed on, and
some consensus of
working together is reached.²
These objectives are achieved by sharing
perceptions of the situation, clarifying the client's and nurse's goals, and
testing plans of
actions that best fit the client's interests, needs, and approaches to
learning. At times
the initiative for planning falls on the
nurse and at other times on the client or family; ideally, however, this
is a shared
activity.

During the implementation phase of the nurse-client relationship, the client's
major task is to
become an active learner. How- ever, the degree to which the client can
become active
in this proCess is contingent on his or her physiological, psychological, and
social status.
The nurse's major role is to tailor the learning activities to the
client's needs
and characteristics. It should be noted that learning, in thi5 model, is
used in its
broadest sense, ie, from the cellular level to the more global
social system
level. The nurse's role is determined by the client's potential for engaging
in health
work. For example, for the client who has suffered a myocardial
infarction, the nurse's
major efforts in the initial stage may be directed toward maximizing the
client's
physiological potential. In later stages, however, the nurse and client
may work on
other areas of health potential.
It is during the implementation phase that the collaborative nature of
the nurse- client
relationship more fully manifests itself. The client takes the lead in
working on
aspects of health such as improving problem-solving skills—for example,
rede- fining
the problem, identifying patterns of functioning, linking solutions to
directions and goals,
increasing repertoires of coping skills, trying out new ways of dealing with
old problems,
and so forth. Furthermore, the client contributes to the process by
establishing the
pacing and timing of the various activities.
In this process, the nurse can play a
variety of roles depending on the client's tasks.m For example, if the
client's task is to
identify, Clarify, or enlarge his or her per-
spective of the concern at hand, the nurse can facilitate this process
by acting as
a focuser or stimulator. Projects can be devised to help the client
gather
informa- tion.
The following example illustrates several ways in which the nurse can
motivate a family to
develop its health potential. The parents in this situation are having
difi- culty
with their child's temper tantrums. The nurse might ask the mother and
father to do a
little homework in the form of observing their child and noting the partic-
ular
círcumstances surrounding the temper tantrums as well as what the child
actually does. The
parents share their observations with the nurse who, in turn, helps them
identify
linkages between events and the tantrums. In this case, the temper tantrums
are observed to
increase before mealtime when both parents are occupied. The par- ents
soon realice
that if one attends to the child's needs while the other prepares the
meals, this might
solve their problem.
First, the choice of the activity, ie,
observing and recording their child's behavior, served to detach
the parents
from the situation. Assuming an observen role, the parents were able to
learn more
about their child's behavior. This experi- ence also captured the parents'
interest and
motivated them to search for a different solution. Second, by the
nurse helping
them to come up with their own solution rather than suggesting one,
the parents
discovered that control ovef the situation resided with them. For the
parents, discov-
ering new problem-solving skills and an increased sense of control served
to rein-

together nurses
in the university setting and nurses in the practice setting. Clinicians,
researchers,
administrators, and educators committed to the model have been responsible for
its continued
growth.
