JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSON

We, ELSA O. VITALES, of legal age, Filipino citizen, with residence address at 4772E Barasoain Street,
Olympia, Makati City, and, ERLINDA S. ISIDRO, of legal age, Filipino citizen, 2020 Samat Street, Olympia, Makati
City, after having duly sworn to in accordance with the law do hereby depose and state:

1. That, we know personally ALLAN DIAZ who was born on 23 April 1974, at Calbayog City, Samar, to
parents Ignacio Tan and Adelina Diaz and later passed away on 19 June 2020, at Batangas, due to
Hemorrage;

2. That, whereas, the said ALLAN DIAZ was born out of wedlock therefore his name shall be registered the
same and not ALLAN DIAZ TAN due to illegitimacy and in accordance to Philippine Law, however, his
name has been entered and declared in other legal documents as ALLAN DIAZ DIAZ;

3. That, we further know that the said ALLAN DIAZ had been married to YOLANDA RAMASASA PABELLO
on 12 December 1997 at Bauan, Batangas, and begot three (3) children whose birth particulars are
written below:

4. That, whereas, the date of marriage of said ALLAN DIAZ was entered as 23 April 1997, at San Pascual,
Batangas, in the Certificate of Live Birth of the second;

5. That, in view thereof, we hereby attest that the name ALLAN DIAZ, ALLAN DIAZ TAN and ALLAN DIAZ
DIAZ refers to one and the same person pertaining to him;

6. That, we further attest that the said deceased person had been married to YOLANDA RAMASASA
PABELLO on 12 December 1997 at Bauan, Batangas, and not on 23 April 1997, at San Pascual,
Batangas;

7. That, we finally attest that the said person left two (2) surviving child whose birth has been registered
before the proper Civil Registry Office;

8. That, we knew all the facts because we are friends of the above mentioned name;

9. That, we are not related to the name mentioned above in consanguinity or affinity and we hereby attest
that the forgoing statements are true and correct to the best of our knowledge and belief;

10. That, we are executing this affidavit to attest to the truth of foregoing facts, for information and
guidance of proper authorities concerned and for whatever legal purpose this affidavit may serve.
