Welcome to the Implementation Assessment Tool (Endline)!


Thank you for all the work you have done in implementing distance learning this school year. As we continue to pursue data-driven and evidence-based decision making, your
serves as the final tool in monitoring your LDM implementation. This tool will help you
1. evaluate the LDM implementation in your school, which can also serve as a planning document in improving your implementation next school year, and
2. consolidate data that the Central Office will use to determine the effectiveness of LDM implementation of public schools across the country.

Instructions
1.The LDM Implementation Assessment Tool contains 2 types of tabs, and should be filled up in the following order. The
