INTRODUCTION
Laga-an Elementary School is located at Barangay Laga-an, approximately 10,6123.3804
in the Municipality of Calatrava, island of Negros. The total land area of this barangay
reaches about 15,830.76 square kilometer. According to the attest enumeration from the
Philippine Statistics Authority, it has a total population of 2,686 with 703 families. The
age group with the highest population in Laga-an is 10-14, with 337 individuals.
Conversely, the age group with the lowest population is 80 and over, with 20 individuals.

The local community and the school adapt to drastic changes brought by this
COVID-19 Pandemic. It shifted to print modular distance learning to accommodate the
diverse needs of learners, teachers and the community.

Moreover, Learning Continuity Plan is designed to enhance the practices of the


school in the new normal and further address the needs and strengths as an educational
institution. As such, education continues without compromising the health and safety of
our learners.

SMART GOAL: By the end of SY 2021-2023, 100% of Laga-an Elementary


School Teachers are encouraged to attend the virtual seminar conference through
online platform for the purpose of enhancing the teachers performance in different area
of concerned, in relation to their assigned task more focus on reading comprehension
and academic achievement /performance of the learns to face the reality of life for this
through ever changing world.

2. ACCOMPLISHMENT REPORT

All grade level class advisers are involved in the distribution and retrieval of module
every Monday. Parents and guardians are the only ones allowed to get the self-learning
modules and must observe health and safety protocols. An orientation and simulation of
the learning modality was conducted prior to the actual schedule of the distribution. A
booklet was also presented which serves as a guide to the parents/guardians.

Ways of tracking learner’s progress:


• Every quarter, the teacher monitors and report the progress of each learner.
• Home visitation is also an option provided that the teacher secures a consent from
the school head and should observe health protocols while conducting a home visit.
• Reading assessment was done by teachers handling Kinder to Grade 3.

• As of SY 2020-2021, there was no recorded pupil/s who got a failing final general
average. But the teacher can give remedial lessons to those learners who needs to
improve in academics provided that the teacher secures a consent from the school
head and should be done based on DepEd guidelines.

INNOVATION:
“Gasa Mo, Basa Ko” – A community outreach program of Laga-an Elementary School
which aims to help impoverished families in far-flung areas by sharing food packs, and
conducting reading sessions and basic tutorials to learners while observing health
protocols.

blood pressure, asthma and diabetes, common causes are undisciplined eating habit
and unhealthy lifestyle of a person.

modular distance learning because of the strict implementation of IATF health protocols.
The school ensures that only parents/guardians are allowed to enter the school.

Learnings from SY 2020-2021


• This world health crisis costs lives of many and greatly affects our health care
system, economy and education.
• The delivery of instruction in the basic education is susceptible to changes because
of COVID-19 pandemic.
• Laga-an Elementary School shifted to print modular distance learning and made
sure that access to education is possible even without face-to-face classes. We
successfully implemented this kind of delivery modality.
• Parents had various feedbacks about the current situation of learning which
suggests them to facilitate their own children at home.
• Teachers made innovations and solutions on how to assess learners in the context of
distance learning including the use of mobile phone and social media to connect
with their pupils and answer their queries.
• Pupils had to collaborate with members of family and seek assistance in order to
answer the learning modules.

cooperation with local government of the barangay.

2.I – AWA Schedule
Revised guidelines on Alternative Work Arrangement in the Department of Education
during the state of National Emergency due to Covid 19 pandemic stringent social distancing
measures from October 2020 to March 2021 every teachers reported 3 days a week on site
while 2 days was Work From Home and from March 2021 to July 2021, every teacher reported
4 days on site while 1 day Work From Home.
2.K – Health Protocols
Covid-19 health and safety protocols in Lagoc Elementary School personnel for
informational purposes and safety, the learners especially in elementary are mandated to stay at
home. Everyone has a responsibility to practice social distancing, always wear facemask and
face shield, frequent handwashing, cough/sneeze etiquette, paper tissue usage and disposal.
Avoidance of touching their face, and respecting others.

3. Learning from S.Y. 2020 – 2021

• The COVID-19 pandemic has created the largest disruption of education system.

• Lagoc ES personnel have to adapt the new methods of educating the learners.

• School personnel staying connected to the parents, stakeholders, SK officials,


and Brgy. Officials.

• School personnel followed the health protocols strictly.


• School personnel have distributed the learning materials to the parents for the
learners and retrieved.

5.C – Mechanics of the Delivery


Lagoc Elementary School personnel would be implementing the Distance
Learning (Modular-Printed) for S.Y. 2021 – 2022. The parents/guardians of the
learners have claimed and returned the modulesprinted in a weekly basis. The method
of distribution and retrieval were done and required health protocols. It is the
responsibility of the school personnel to ensure the safety in the provision of modules-
printed to the parents/guardians taking into consideration of the significant cases
COVID-
19.

9.A Priority Group

The safety and well-being of learners, school


personnel, and parents/guardians remains our top
priority as we continue to put learners in new normal
education. School personnel have strategies in place
to enable distance learning so we can continue to
focus on learning and our commitment to our
learners and their families. Having effective plans in
place, we minimize disruption, condition
and uncertainty for our learners for S.Y. 20212022.

The school personnel strictly implements the rules of the IATF including the health
protocols inside the school premise. Everyone must observe the following:

Assessment of learning have some of the most essential consideration for
how assessment may need to change given the dynamics of the 2021 – 2022 school year
is absolutely modules-printed. The type of learning activities should occur minute by
minute and day by day so that educators can use evidence of learning to adapt
instruction in real time to meet the learners’ immediate learning needs. 10. B

C.2 Parents Involvement


Lagoc Elementary School parents involvement always mattered. Parents have seen
known to be a child’s first teacher for the moment a child is born as they mature and
become a learner.

All stakeholders assured their involvement to Lagoc Elementary School continuously
support education to the learners in the new normal. The school personnel have the
obligation to disseminate necessary information for the incoming S.Y. 2021 – 2022.

C.5 Partnerships
The importance of a healthy and strong partnership between parents and school
personnel is well-recognized and have proven that collaboration between the parents
and teacher improve learners academic achievement.
C.6 Safety Contingencies
The Lagoc Elementary School situated at Barangay Menchaca,
Municipality of Calatrava, where there is a safety contingency in the health center that
implement programs, projects and services in emergencies in this time of pandemic for
new normal.

• Align with harmonized National Contingency Plan for the emerging


Infectious Diseases (COVID-19) of the National Disaster Risk Reduction
Management Council (NDDRMC).

• Establish the coordination and communication mechanisms among the


LGU, RHU, District and Division Offices and the affected and the
assisting school;

• Set the response and rehabilitation or early recovery roles and functions
of the DepEd offices and the affected and assisting schools for the
purposes of education continuity;

• Provide guidance for the continuing capacity building of DepEd officed


and schools;

• Determine the preparedness and prevention and mitigation measures to


reduce the impacts of the emerging in infectious Disease (COVID-19); and

• Support the identification of needed equipment and supplies to reduce the


impacts of the Emerging Infectious Disease (COVID-
19).

III. MONITORING AND EVALUATION


1.A SMEA
On the S.Y. 2021-2022 Lagoc ElementarySchool has probably one male PARDO from
Grade V S.Y. 2020- 2021.
But at the end of S.Y. 2021-2022 on July 2021, Lagoc Elementary
School would be no/zero number of failure.

1.B SCHOOL HEAD OBSERVATIONS


As a School Head of Lagoc Elementary School, I have observed on the priority areas
on the Access that there was an increased of Enrolment for the S.Y. 2020-2021
compared as of last S.Y. 2019 – 2020 for Kindergarten of 1.75%, however there was a
decreased of Elementary enrolment of (.90%).
On the S.Y. 2021-2022 probably with an overall decreased of
(.66%) enrolment compared to last S.Y. 2020 – 2021 enrollment.

School Health Protocols


School health protocols was strictly implemented based on IATF guidelines. No learners
were allowed to enter the school premise. Parents/Guardians wear facemask, face shield
and frequent handwashing as required by the DOH.

Lagoc Elementary School is located at the, remote area, therefore, there are
parents living in kilometers away from the school.
The parents were sometimes hesitant to get/return the printed
materials/modules to the teachers in school. In this scenario, the teachers were
obliged to home visitation despite the fear of COVID-19 pandemic or health
issues.
