Re: Ms Betty Johnson, 81 years of age

I am writing to update you regarding your patient, Ms Johnson, who is being discharged today after a
successful recovery from total knee replacement surgery. Your further follow-up would be highly
appreciated.

On 25/02/2015, the surgery was performed and Ms Johnson's condition postoperatively was stable.
However, she had developed a severe anemia; for which, an intra-venous blood transfusion was done.
After controlling her Hb level, Feratab was commenced. In terms of her medications, five days
preoperatively, warfarin was ceased and was replaced by clexane. The later was ceased when her INR Commented [jc1]: Clexane
became elevated and warfarin was restarted again three days postoperatively. Regarding her wound, it has Commented [jc2]: latter
healed well without oozing and clips have been removed since the surgery.

On 06/03/2015, Ms Johnson was transferred to a rehabilitation unit where her mobility improved
significantly after doing gentle exercises. Now, her independence is increasing gradually as she is able to
move with minimal assistance. Therefore, she is being discharged today with a home nursing assistant.
Apart from that, her medication list includes warfarin, Feratab, paracetamol and oxycosone if required. Commented [jc3]: oxycodone

Given the previous data, I would be grateful if you could follow-up Ms Johnson and monitor her FBE and
