POLICIES ON THE USE OF

CHEMICAL ENGINEERING
LABORATORY
DEFINITION 1. Instructional laboratory classes
include ChE 157, ChE 158, ChET
122.1, ChET 142.1, ChET 151.1,
ChET 161.2, EnvET 105 and EnvET
115.
2. Research laboratory classes include
ChE 194, ChE 195, and EnvET 199
DEFINITION 3. The regular working hours are
8:00AM–5:00PM, Mondays to
Fridays, excluding holidays.
4. The supervisor may be any faculty
member, lecturer, research aid, and
laboratory technician of the
Department.
DEFINITION 5. Lab Form 1: Laboratory Use Permit is
issued by the department to allow research
lab students to work in the ChE labs, and in
the irrespective Research Labs.

2. Lab Form 4: Waste Management Plan
3. Waiver
4. Photocopy of accident insurance.

To be accomplished at the start of the semester,


or for first-time use.

LAB FORM 1: LABORATORY USE PERMIT

DEFINITION • Prepare 2 copies. Regular working hours are on weekdays only.


• Work beyond regular hours (8:00 AM – 5:00 PM) requires the presence of a supervisor and an
approved LAB FORM 2.
• This form is to be submitted at least two (2) working days prior to the start of work.

6. Lab Form 2: Permit to Work Beyond
DEFINITION Regular Working Hours is a special permit
issued by the department to allow research lab
students to work in the ChE lab, and in the
respective Research Labs beyond 8:00 AM to
5:00 PM, Mondays to Fridays.
Attachments:
1. Lab Form 1: ChE Laboratory Use Permit
2. Lab Form 3: Risk Assessment Form
3. Approved Institute Security Permit for issuance of
Authorization Pass.

INSTRUCTIONAL Extension beyond class hours
LABORATORY • Students taking Instructional laboratory
classes are allowed to extend beyond
the regular class hours under the
supervision of their instructor.
• Students shall be allowed to conduct
make-up experiments ONLY upon the
approval and under the supervision of
their instructor.
UNDERGRADUATE Laboratory work in Research
and GRADUATE Labs
RESEARCH • Students doing Undergraduate Research
are allowed to conduct experiments in the
Research Labs during regular working
hours provided that they have an
approved Lab Form 1 (to be
accomplished at the start of the semester).

• Lab Form 1 (with necessary attachments)
UNDERGRADUATE Work beyond Regular Working
and GRADUATE Hours
RESEARCH • Students who will extend their work in the
laboratory beyond working hours not
exceeding 9:00 PM, or weekends, need an
approved Lab Form 2 and should be
accompanied by a supervisor.

- Approved Lab Form 2: Permit to Work
Beyond Regular Working Hours
UNDERGRADUATE Work Overnight, on Weekends, and
and GRADUATE Holidays
• Students who will be working in the Research Labs (i) beyond
RESEARCH 9:00 PM, (ii) Weekends, or (iii) during holidays, should be
accompanied by a supervisor and need to accomplish the
following:

2. Approved DSA form request to the Institute Security.
• Failure to follow general laboratory policies, neglect of
DISCIPLINARY laboratory responsibilities, and doing any action that may
ACTION endanger you and other people using the laboratory will
warrant the following disciplinary actions.

• First offense: Reprimand from the Department Chairperson,


letter of apology addressed to the Department Chairperson thru
the Laboratory Coordinator, 10 hours mandatory
lab/department service.
Second offense: Reprimand from the Department
Chairperson, letter of apology addressed to the Department
Chairperson thru the Laboratory Coordinator, 20 hours
mandatory lab/department service.

• Third offense: Reprimand from the Department Chairperson,


letter of apology addressed to the Department Chairperson thru
the Laboratory Coordinator, 30 hours mandatory
lab/department service. 1 week suspension from using any lab.
