DECLARATION OF CONSENT FOR ONLINE PROCTORING

The University of the East Ramon Magsaysay Memorial Medical Center – College of Allied Health
Professions takes the protection of your student data seriously. As a matter of principle, the
protection of your individual privacy and personal information, is of the utmost importance to us.
For this reason, compliance with the legal provisions of Republic Act 10173, otherwise known as
the Data Privacy Act of 2012, is a matter of course for us.

For online exams proctoring, we will use videoconferencing applications using the student’s
webcams, tablets, or smartphone cameras. This can be conducted in the form of online live
proctoring or online record and review proctoring. Online live proctoring means live monitoring will
be done by proctors who are faculty of the College of Allied Health Professions, using the
students’ webcams, laptops, or smart phones. Online record and review proctoring, on the other
hand, means recording of the examination situation using the students’ webcams, laptops and/or
smartphone cameras with subsequent evaluation by the faculty of the College of Allied Health
Professions.

For this purpose, video and audio recordings of the examinee are made for identification purposes
and during the exam, as well as of the room and workplace and the screen or the device to be
used during the exam to exclude any attempts at cheating. The data recorded will be deleted
within one week after the examination. In case of any suspicion of a fraud attempt, the data will
be stored until the investigation of said event is complete.

Participation in online examinations, in which supervision is carried out either by online live or
record and review proctoring, is voluntary for you and is based on your consent. If you
nevertheless decide (by not consenting to online proctoring) against our offer to take an online
written exam today, you may be offered the opportunity to take the exam during the make-up
period, or face to face at the UERMMMCI when the quarantine status permits, in an area so
chosen by the faculty. You can change or revoke your declaration of consent at any time without
giving any reason for your revocation. If you revoke your consent during an online assessment,
the online proctoring will be stopped immediately, and you will also stop taking your examination
to accommodate for alternative measures for you to take the examination as earlier mentioned.

the aforementioned online proctoring, making, and storing of video and audio recordings for the
examinations I will undergo for School Year 2021-2022 for the BS Medical Technology program
of the College of Allied Health Professions
