A-

Today I did six main things. First, since I was assigned to a new patient today, I began by
establishing a rapport with her. I did this by introducing myself and by making small, casual, and
general conversations with my patient. Next, after I gained my patient's trust, I took her vital
signs, as well as the vital signs of her baby boy. While briefly interviewing my patient, I figured
out that she was just in the taking-in phase of the postpartum period. Being in this phase, she was
still recovering from her recent delivery, and all she wanted to do was talk about her experience
during labor and during delivery. So, the third thing I did today was to listen to, and encourage
my patient to share her experiences during labor and during delivery yesterday. Fourth, after my
patient left to comply with her requirements, I assisted my groupmates in bathing a baby. While
they were bathing the baby, I provided the first-time mother with health teachings on the proper
way to bathe her baby at home. I mentioned and explained each step of bathing to the mother at
the same time my classmates were performing it. Next, at noon, I again took the vital signs of my
patient and I plotted her vital signs on her chart. Finally, since my patient complained of being
exhausted from not sleeping, I performed basic nursing interventions to help her get some rest.
All of these interventions were documented on my clinical portfolio’s nurses notes (FDAR).

R–

Today I felt, learned, and realized that I have so much more to learn about the specific tasks in
the OB ward. I realized this since most of what I did today were all accomplished within the first
half of my shift. The second half of my shift honestly felt unproductive since my patient was
very busy with her requirements I was not able to provide her with bedside care. I also felt that I
have so much more to learn since a patient asked me to “fix” her IV line for it was full of blood
and all I could do was refer the case to the nurse on duty. Another patient also asked me a
question on why eclampsia caused the death of her first born long ago, and I felt bad not
knowing the answer to her question.

A – 

As a response to my realization, my action is to read in advance about some of the basic concepts
accompanying pregnancy and delivery with complications. I know these concepts are taught next
semester, however, I also know that reading in advance will better equip me to, at the very least,
be able to explain to a patient what is going on within her body.

S- 

In today's session, I mainly contributed my interpersonal skills. I did this by providing the first-
time mother with health teachings about the proper way to bathe her baby. In synthesis, today’s
session honestly felt a little unproductive in terms of one is to one nurse-to-patient care, since my
patient kept leaving her bed. Hopefully I learn from today’s session and be a lot more productive
tomorrow.
